A series of education training of documentation was implemented to help reduce episodes of Medicare payment denials and self-protection through adequate documentation. Thus, I will discuss the impact of inadequate nursing documentation that leads to malpractice lawsuits. Purpose of Medical Record Documentation Understanding the purpose of medical documentation was the first step in teaching how to prevent inadequate documentations that leads to liability and malpractice lawsuits. Monarch (2007) supports the purpose of Medical Record Documentation as the following: • Substantiating the health condition or illness or presented concern for the patient. • Effective communication among health care staff.
More often a nursing assessment is based on the medical side of the patient rather than the holistic approach. In this assignment I will be discussing the importance of the nursing process, care planning, and looking at how these are used in practice. I will look at the tools used in the nursing process and show an understanding of how effective they are when used correctly. I will achieve this by describing a case study of a patient from my practice area, and discussing two specific areas that affect the patients care. Throughout this assignment I will be using a pseudonym to maintain patient confidentiality in order to conform to ‘The Nursing and Midwifery Code’ (NMC, 2008).
ASSIGNMENT TITLE: AN APPRAISAL OF A PIECE OF RESEARCH-BASED EVIDENCE Evidence based practice (EBP) is the term used to describe how healthcare professionals base their clinical approaches on reliable evidence, to ensure the best possible outcomes for patients. EBP encompasses significant and trustworthy research based evidence, patients own preferences to their care and the expertise of healthcare professionals involved, to determine the optimum care for the patient (Rees, 2012, p.9). It is extremely important to professional practice, as the Nursing and Midwifery Council (2010, p.6) states that nurses and midwives must: ‘deliver care based on the best available evidence…’. To find relevant evidence, a succinct question must be created using a standard framework to provide appropriate keywords for an efficient search (Davies, 2011, p.75). The topic under review is hand hygiene and face mask effectiveness in the spread of influenza-like illness.
Although the first years of a baccalaureate program are similar to that of an associate degree program, additional in-depth coursework such as social sciences, nursing research, public and community health, nursing management, leadership and critical thinking are required, thus enhancing a BSN’s professional development, knowledge base and scope of practice. A BSN program focuses on treating patients as individuals by incorporating the metaparadigm theories of nursing, allowing for a higher level of preparation to care for more complex patients. A BSN is trained and prepared to treat each patient holistically; assessing, evaluating and analyzing all needs (physical, emotional, environmental) of the patient. Although an associate-level degree nurse has some limited knowledge of theory and conceptual models, their training primarily focuses on technical skill and task orientation. The ADN has a disadvantage when it comes to the concepts of health, environment, person and nursing as well as evidenced based practice.
Quality Improvement (QI) can lead to a general approach to the measurable improvement of patient care. Quality improvement uses specialized plans and principles that guarantees quality of care for patients in healthcare facilities. Quality outcomes centers on quality management principles. This report will cover the fundamentals of Quality Improvement. This report will also explain the importance of stakeholders an how quality is identified.
The call to form the primary care workers, including advanced practice registered nurses, especially nurse practitioners. The call to build the primary care workforce, will increase as access to coverage, service settings, and services will increase under ACA. Nurses are good in assisting transition from hospital to home, preventing medication error and reducing rates of infection. In short the product of the report if followed, would rise the number of nurse practitioners and doctorate level nurses to deliver health care treatment for some patient who are looking for treatment and are unable to find a health care provider ready to take
Quality Improvement Part ll HCS/548 Mary Ellen Strout October 21, 2013 2. Quality Improvement Part ll Quality improvement is a hospitals applied process in order to ensure the advancement of the quality of care and outcomes for patients using an explicit set of philosophies and procedures (Walker, 2012). There are potential advances for quality improvement at Jordan Hospital in the Emergency Department. The quality improvement that would be applied uses the measurement of the collected data to be used towards the strategic improvement of patient care. Using the measurements taken and applied tools, it allows for leaders to understand the direction of the quality management in an organization or facility.
The role of expert system in nursing care and medicine Nursing care and medicine rely on expert systems to augment their professional ability to carry out their day to day patient care. In today’s patient care environment, many nurses are already technology junkies. Cipriano, 2011, p.289. states that "Technology is the greatest tool available to transform and innovate the delivery of nursing care". Solutions exist today that can eliminate repetitive and mundane tasks, accomplish regulatory work, provide more efficient use of space, improve the environment of care, provide access to resources, and thereby improve safety and efficiency.
Training and Education in Health Care Jazmin Ontaneda HCS/341 March 5, 2012 Dorothy Cucinelli Training and Education in Health Care 1.Explain why training and education are vital in health care The health care industry is as diverse as the persons it serves. Constant change in medicines, procedures and mandates cause the need for higher education and continual training. Education and training provide the foundation for a knowledgeable staff. The expectation of life-long change within the health system develops a staff ready for change culminating in adaptation of diverse situations with each individual patient need. Training and education are vital in the healthcare field because it determine the quality of care that each provider or administrator will deliver.
Discuss the importance of research in health and social care To support care information gathered from research can be used by health care proffesionals so they can give the best possible care and treatment. It is important to ensure that the right people have access to the right information at the right time so they can provide you with the right care. To improve health and social care services for everyone Information is used for purposes beyond your direct care: Commissioning The people who plan health and care services (commissioners) need good information about the types of illnesses people have and the treatments they receive, as well as the result of that care or treatment. They can then check to make sure that people are getting the services that are right for them. Public health Some information is used for public health.
