1 Family Case Study 1. Client Identification and Intake Information The identified patient is Tristan, a 15-year-old Caucasian boy who lives in a middle class neighborhood with his mother, sister 10-year-old brother, Jonathan and his 1-year-old sister, Ava. Tristan’s father is part of his life but he has not slept at his house in over a year. Tristan just turned 15 three weeks ago and is a freshman in high school. 2. Identified Medical and Psychiatric Conditions Tristan’s medical and psychological history has been normal to date. Tristan’s grandfather on his mothers side was chemically dependent and some psychological problems. On his fathers side there is depression with both parents. By the end of 8th grade he was no longer attending school because he had fell to far behind. Tristan by 8th grade was completely enmeshed with his mother and she was now enabling him by making excuses for him about his father’s abandonment. Tristan’s father was unaware of everything because he was dealing with his alcoholic girlfriend at his house. Tristan’s mother believe he was acting out due to his father choosing his girlfriend over him. Madison also thought it was due to hormones and allowed the behavior to continue. 3. Family Interview Tristan Donley-15-year-old IP: Tristan describes himself as shy, easy to be around, has a temper and is easily angered when confronted about his character. He’s always done good in school only because it came to him and that’s the reason he does’t like to go because he’s not learning anything. He hates his brother because he’s annoying and always getting into his business. He thinks his father spoils him at that also bugs him. Tristan also believes he like doing drugs because they are fun not because he’s escaping. He doesn’t understand what the big deal is it’s his life and body he should be able to do what he wants with it. Todd Donley- Father (Enabler): Todd is 39-year-old caucasian man. Todd has been a media buyer at the same company for over 12 years and has been lead buyer for 5 years. Todd went to college but never graduated. Both parents will have same rules posted at house and deal with consequences accordingly. They will also set up chores for Tristan so that he may gain his trust back and obtain his independence with these new strategies. 3. Parents will communicate regularly and be supportive of each other knowing that recovery is a process and needs constant attention. 4. Mother will follow through on punishments and be authoritative with Tristan to regain control of her household. 5. Tristan and Jonathan will make plans to do activities to help with their brother bond so that Jonathan does not fear brother. Tristan will attend Jonathan’s baseball game and be supportive of brother. 6. Tristan will start school and attend on a regular bases. Tristan will meet with counselor to set up contract that he will remain sober and attend school with no behavioral problems. If he is have any issues he should meet with counselor. 7. Father will take both boys to do actives that they can bond like fishing or baseball games.
