Planning for Practice Change The Pool Activity Level Tool (PAL) Introduction My background in care began in psychiatric nursing. While training I felt I was not able to uphold my principles and values concerning the care of vulnerable people, which includes, valuing the person, treating them as an individual, and building relationships that form the basis of supportive care. I was encouraged to view patients as a set of signs and symptoms needing to be managed. Interaction was limited to “protect” the nurse from getting too involved and to keep control. I could not make a difference, so I left. I became a domiciliary care assistant. I could see the difference my involvement made, which was both motivating and rewarding. I moved into management hoping to implement my principles and values, however, this was not the case. I had to uphold barriers, focus on the task itself and cut times to increase visits. A sample group were chosen consisting of those who display “distressing” behaviours on a regular basis but are labelled as being “aggressive”. A common thread emerged which gave rise to a need for more meaningful interaction and occupation and for them to feel that they belonged. As one lady said, “We need help in finding purpose to our days. There is nothing to do here and we feel we don’t belong”. Meaningful occupation is central to our well being so that boredom and apathy do not have a chance to set in (Downs and Bowers 2008). It reinforces skills, gives purpose and expresses the personality of the person with dementia (Brooker 2007), therefore, knowing how the residents feel about their care can result in a positive change towards a meaningful
