The windshield survey prompted me to view what a client is from a different angle. As a floor nurse my comfort zone is one of working with one person, or sometimes a small family unit one at a time. My usual interventions and goals are focused on success for this person, in the way the individual defines success for themselves. To begin my project I had to first determine what is meant by community, community health and having your client as the whole community. Ways to define community can include: • A network of people who have a commonality who are supportive of one another’s interests, for example the elderly community. • People who share living in a common local area, for example the community of the Pearson Neighborhood.…show more content…
The recipient of care through interventions and education is comprised of a group of people. Community as a client differs from an individual client. The assessment and determination of needs is based on a whole group, versus a lone individual. Of course a community is made of individuals, but the focus is one of macro versus micro proportion, (Stanhope & Lancaster, 2008). After introducing these terms to myself, I set out on several tours of my area and also made notes throughout my week while commuting about the area. This provided the data for my windshield survey at the end of this reflection portion. I started to see everyone as clients, wondering where they went home to, what kind of family they came from and what their health challenges included based on their age, circumstances, place of dwelling, and beliefs. Community factors that could affect our population’s health included: Positive effectors: • Community’s location in a warm area near a small beach. • Availability of a Senior Community Center. • Reputed low tolerance of community police force, thus enhancing a safer environment. The one selected is reducing the rate of emergency department visits due to falls among older adults with a ten percent improvement, (Healthy People, 2011). To succeed this health indicator could benefit from the efforts of community nursing through partnering with local businesses, provision of community education, and networking with local health services. Community Nursing Diagnoses that could help focus the efforts of the community health nurse with the stated health indicator include: • Risk for falls, among older population, related to lack of screening and lack of education about prevention of falls as evidenced by increased ratio of older adults versus other populations that experience injury sustaining falls. • Ineffective community therapeutic regime management, among older population, related to lack of access to fall screening and education about causes of falls as evidenced by amount of emergency room visits in 2010 of older adults seeking emergent care related to
