Outcome 1 - Understand the application of person centred care approaches in health and social care 1.1 person centred values must influence all aspects of health and social care work because Person-centered is about providing care and support that is centered or focused on the individual and their needs. We are all individual and just because two people might have the same medical condition, for example, Dementia and learning disabilities, it doesn’t mean that they require the same care and support. Developing a clear understanding about the individuals we are working with. Will help carers understand their needs, their culture, their means of communication, their likes and dislikes, their family and other professionals’ involvement so we can promote and provide person centered care and support. Person-centered values provide a foundation on which we can base and build our practice.
Assignment 206 Task B Bi 'Agreed ways of Working' The policies and procedures written by the employer set out how you should provide care and support for others and how you should deal with specific issues in your work setting. These should include policies and procedures relating to health and safety, equal opportunities, confidentiality, data protection, supervision, waste management, moving and handling, handling medication, and security and safeguarding. These should identify the general approach that the company takes towards an issue, they should incorporate all of the legal requirements affecting care work and should reflect the safest and most effective ways of carrying out particular tasks. It is essential that you understand and follow these policies and procedures to ensure that you are working in ways agreed by your employer. The duties and responsibilities of your role as a social care worker should also be set out in your job description.
INTRODUCTION TO DUTY OF CARE IN HEALTH, SOCIAL CARE 1.1 As a care worker, you have a legal obligation to adopt a ‘duty of care’ approach towards the people within your service. You are required to work in the best interests of the people using the service which includes maintaining their safety and wellbeing. 1.2 It is important to carry out duties that are in own job description and that you are competent. It is also necessary to regularly update own knowledge and skills in order to uphold and provide the care standards that are in accordance with the codes of practice of the care setting. This includes the use of any available resources such as hoists for the moving of residents and ensuring training is up to date by attending all relevant courses.
These include people such as: Social Workers, Physiotherapist and Nurses etc. The main and important statement is to make sure everything is done according to policies and procedures and according to the government law. 1.2- Explain expectations about own work role as expressed in the relevant standards My expectations are to progress further with my career, increase my knowledge, experience and understanding. Unit 4222-305 Promote person centred approaches in health and social care (HSC 036) O1 Understand the application of person centred approaches in health and social care O1.1 Person centred values must influence all aspects of health and social care work as it is about building everything you do around the individual you are working with. It is about enabling the individual to take control of the planning and delivery of their own support and care system. The key areas of person centred practice are: * Practical and physical support – the day to day living such as personal hygiene, dressing, preparing meals, domestic tasks, shopping and social activities. * Emotional support – take time to talk to the individual and take an interest in what they have to say offering understanding and empathy if needed and listen effectively giving them time to express themselves and always respect their feelings and views. Support and encourage the individual in their choices and give support to help them overcome any difficulties they may encounter.
If person centred values that underpin all work in the health and social care sector are followed as they should be then all individuals should feel that and health and social workers ensure that the individual is supported in accessing their rights, they are treated as an individual, they are supported to exercise choice, they have privacy if they want it, and be treated with dignity and respect. To ensure this is followed and influences all aspect of health and social care its important to ensure individuals are treated as such and person centred care should ensure this. Evaluate the use of care plans in applying person centred values Care plans are the basis for how social care and support services are delivered. The most important part of the plan is the outcome that the individual wants to achieve. One of the key ways in which care plans reflect person centred values is that the service provider does not structure services around what the providers do in order for individuals to receive services.
Identify the different reasons people communicate People communicate in order to give instructions, to acknowledge they have understood an instruction, to share information, to build relationships with people and to share opinions, knowledge, emotions and to give encouragement and advice. As a carer I am required to communicate with colleagues, clients, client’s families and medical health care professionals whilst at work. I am required to communicate effectively with others to ensure I am meeting the needs of the client, when booking appointments for the client with medical professionals and with the client’s family about any quiries and updates they require about the client. I am also required to communicate regularly in writing when carrying out tasks such as documenting information in communication books and various charts such as food and fluid intake monitoring, documenting any medication that has been handled and filling in timesheets. You will have to show you are competence in doing your job role and meet the standards that are required for you to carry these out there is The National Minimum Standards these are used by the Commission for Social Care Inspection to inspect the quality of care in services, National; Occupational Standards UK standards of performance that people are expected to achieve in their work, and knowledge and skills they need to perform effectively. 2. Be able to reflect on practice. 3.3 Explain the importance of reflective practice in continuously improving the quality of service provided.
