Understand the application of person centred approaches in health and social care Explain how and why person-centred values must influence all aspects of health and social care work. A: A persons centred values are there own individuality , there own rights and choices. The right to privacy and independence , dignity and respect. Individuals have an equal partnership. Evaluate the use of care plans in applying person-centred values. A: The use of care plans within a working environment is an outline of the individual needs of each person. This is were there daily requirements are written any preferences that an individual requires for all personal care or assisted care is required and how much care is needed to help the individual on a daily basis. Outcome 3. Be able to establish consent when providing care support. Analyse factors that influence the capacity of an individual to express consent. A: Consent means that an informed agreement has been made to a decision or any action that has been agreed with an individual or the individuals family. Or consent may be given by an advocate who works on behalf of the individual. Explain what steps to take if consent cannot be readily established. A: If consent cannot be made by the individual themselves then you may need to speak to the family or friends. Other members of staff that have worked with the individual may be able to help and give you guidance. Outcome 4. Be able to implement and promote active participation. Describe different ways of applying active participation to meet individual needs. A: Listening openly about the individuals needs. Do not be judgemental about others needs. Never impose your own values and beliefs before others. Always respect the rights of the individual and
