During the assessment phase the nurse will continuously gather data about her patient. For example taking vital signs, observing breathing patterns and monitoring blood glucose will give us internal details about how the patient is doing. It helps midwives provide the correct and safe care for women in their care. The process of record keeping is every bit as important as hands on clinical skills to helping maintain woman's safety. It is not only important for monitoring a woman's treatment and medical condition, it is also important for any legal issues that may arise (Wood 2010). A main point in the NMC 2009 guidelines for good record keeping states "you should record details of any assessment and reviews undertaken and provide clear evidence of the arrangements you have made for future and ongoing care. This should also include details of information given about care and treatment" (NMC 2009).
Also there has been an increase in same day surgery where the patient goes back home following surgery. Patients need follow through visits which may only be done by Visiting Nurses. This aspect of nursing helps to reduce the cost of medical bills. The Visiting Nurse helps ensure that the patient’s condition is improving and coordinates with other healthcare team members to bring the patient back to a healthy stable condition. Visiting Nurse also provide services to patients with such ailments as heart conditions and high blood pressure by helping to monitor their blood pressure and also by teaching them the activities and nutrition practices that would help to keep their blood pressure under control.
Step three would be planning and the outcomes of the patient’s diagnosis, the nurse must do some rapid changes in the patients care to make sure they get treated. Step four consists of the nurses implementing the patients planned health care plan to ensure full recovery of the patient. Last but not least is step five, which incorporates evaluation of both the patients and the nurses’ health plan for the patient (“The Nursing Process”). Just like the nursing process, the scientific process includes five steps as well, with step one being asking a question for the problem being faced with. Step two consists of making a hypothesis to what is to what the answer to the question is Step two would be to develop a study design in order to solve the question Step four is testing the hypothesis with an experiment and analyze the data gathered to see if the hypothesis is.
The student nurse will reflect on health promotion issues as this stood out to me in several areas, The student nurse discovered that although we can nurse a patient back to health it is so important to make the patient aware of how necessary it is for them to participate in their care by adhering to what is recommended to them by various members of the Multidisciplinary Team and the devastating consequences that the patient can encounter if advice is ignored. The student nurse will use the Gibbs (1988) reflective cycle, this encouraged thinking systematically about all phases of an activity. Gibbs (1988) is descriptive with regards to feelings, evaluation, analysis, conclusions and action plans, therefore ideal for this assignment. Although I experienced many different aspects of care regarding health promotions this assignment will concentrate mainly on a single event that will stay with me throughout my nursing career. According to Scottish Government (2010) Healthcare Quality Strategy for NHS Scotland, nurse’s carers and people at the heart of the healthcare and care delivery services should improve conditions in NHS Scotland and the care provided, on reading this strategy I understand a nurses responsibility, but the importance of patient participation in their own care is invaluable and the cooperation and communication with the Multidisciplinary Team is essential to complete patient care package to the highest standard.
