Our unit philosophy is to provide specialist stroke care and rehabilitation to these individuals. This is delivered by a holistic multidisciplinary team, in order for the individual to get to an optimum level of recovery that will facilitate a safe discharge. Although ....... ward is a multidisciplinary working unit, my focus for this assignment will be within the nursing team. The nursing team is made up of 15 registered nurses and 25 non-registered health care assistants and house-keepers; that have recently been merged together from two previously existing wards. My role within the team is that of senior staff nurse, I am responsible and accountable to my ward manager and together we lead the whole nursing team of 40 people.
The biggest difference between the two centers is that RCCHC focuses on preventative and monitoring care using technology. Patients in the network can take daily readings of blood pressure, pulse, weight, etc. from home. The devices used to take these readings automatically send the results to the Health Center. Nursing Profession in the United Kingdom: The nursing profession in the United Kingdom has a long history that traces back to the era of Florence Nightingale who is credited for establishing nursing schools in late nineteenth and early twentieth centuries. During this period, the independence and professional class of the nursing role increased to independent practitioners from its traditional handmaiden role to the doctor. Throughout its, history the nursing profession in the United Kingdom has undergone through several changes in role and regulations. Currently, nurses can work in various settings including health centers, hospitals, patients’ homes and hospitals. Most of the nurses in the United Kingdom are registered with the Nursing and Midwifery Council and work with the National Health Service.
My practice placement was situated in a Primary Care Liaison Team within a community mental health setting. This is a multi-disciplinary team which links with General Practitioner surgeries, inpatient and day treatment services. The team aims to provide a responsive service that assesses, treats and co-ordinates care in the recovery of adults with enduring mental health problems (NHS trust, 2005) who have potential to improve their quality of life. This team is part of four community mental health teams (CMHT) within its Primary Care Trust (PCT), one of the six PCTs within the counties NHS trust, which provides all mental health and learning disability services within this area (NHS trust web) For the purpose of this report, change management is the main theme relating to the setting. This will relate to a strategy developed by the NHS trust in question, to develop it service provision.

It’s through empirical knowing. Empirical knowing is the ‘science’ of nursing, it can be systematically organized into general laws and theories that are used to describe, explain, and predict (Carper, 1978).Empirical knowing is knowledge obtained from school lectures, text books and journals. Empirical knowing focuses on evidenced based practice which leads to effective nursing practice.

Presently I am working in 27 beds of General Paediatric Unit with multi–national medical and nursing staff including supportive staff such as unit assistance, patient care technician and cleaners. My clinical practise provides an extremely encouraging and supporting environment. The mission of nurses in my unit is to provide the highest level of quality nursing care for the children through evidence-based practice. In support of this mission, the nursing team is committed to the delivery and promotion of continuum of exceptional holistic, culturally sensitive and compassionate patient care in timely manner. As part of my career development, I challenge myself by taking a responsibility to be a mentor to a new staff nurse as agreed by my nurse manager.
I am currently a Registered Nurse in the telemetry unit where most of my patients range from middle age to the elderly with heart-related conditions. My duties and responsibilities includes assessments of patients, creating necessary nursing diagnosis, care planning, nursing intervention within my scope of practice, evaluation of the provided treatments, and notifying the doctor when necessary. In addition, I also provide continuing education to patients upon admission, during hospitalization, discharge, preventive measures, treatments, self medication, and home care. As a patient advocate, I will continue to provide compassionate and quality nursing care to my patients. My long-term goals are to obtain a Master’s degree in Nursing in the next five years and to become a practitioner.
BARRIER NURSNG INTRODUCTION This commentary will be used to help everyone to understand what Barrier nursing means and the reason for preventing the spread of infection. However I will be discussing the national and local policies influencing the experiencing of health and social care in nursing. I will also explain the concept of professionalism in accordance with nursing and midwifery council’s (NMC) professional code of conduct (NMC 2009). According to the Department of Health (2004), they explained that the term "barrier nursing" is given to a method of nursing care that has been used for over one hundred years when caring for a patient known or thought to be suffering from a contagious disease such as open pulmonary tuberculosis. However there are two types of infection control barrier and isolation nursing (Garner 1996).
