Evaluation Plan Beauty Sama Grand Canyon University NRS-441v Instructor: Dr. Joyce Morrison (Drj) Evaluation Plan The writer’s PICO question will change the policies and procedures in the hospitals in ultimate goal to reduce CAUTI in patients. This change will affect the outcome of patient care, with that in mind there is great need to evaluate the plan, making sure that although it is an administrative interventions all areas of this change is examine for patient safety. In the proposed solution, there are independent variables (see Appendix A) in the research are; materials used for catheterization, policies and procedures for catheter insertion and removal, and nursing and professional staff. The dependent variables,
QI Plan Part II Virginia Ivins HCS/588 January 12, 2015 Dr. Debbie Simmons QI Plan Part II Quality improvement takes many shapes and forms when implementing a good QI plan in an organization. Kingman Regional Medical Center located in Kingman, Arizona has such plans in place. Quality improvement areas that are important are that of customer service and satisfaction, emergency waiting room times and discharge. Data collection has been an important method and tool for quality improvement in health care organizations. Quality improvement is a process of measuring and improving services that can help leaders understand the direction of quality the organization needs to be successful with its overall service.
Placing patients close to nurse’s station or offering frequent toileting can also help reduce potential injuries. If falls continue to occur with precautions in place, incidents need to be reviewed to identify where improvements can be made. Educational opportunities can be offered to staff on the importance of enforcing fall precautions on at-risk patients. Many different nursing-sensitive indicators can be tracked in the same fashion. The important element here is to continuously monitor standardized performance measures and assess the extent to which nursing interventions have an impact on patient safety, quality, and the work
RTT Task 1 Dede Storms WGU Organizational Systems and Quality Leadership 734.3 Kevin Sauls December 26, 2014 RTT Task 1 A. In this task I was asked to discuss how nursing sensitive indicators could have helped the nurses in the case scenario in identifying issues that may interfere with patient care. Nursing sensitive indicators reflect the structure, process, and outcomes of nursing care. (American Nurses Association, 2014). Indicators of structure are measures by the supply and skill level of nursing staff as well and the education and certifications of the nursing staff.
• To ensure health and safety throughout the surgical procedure and to ensure all instruments, equipment and swabsare are accountable throughout the procedure. • Act as a link between the surgical team and other parts of the hospital so communication and management skills are needed. • In the recovery phase ODP’s are involved in all aspects of patient care in the recovery unit, including monitoring, supporting them and providing treatment until the patient has recovered. Then to assess the patient to evaluate whether they can return to the ward. • Then to evaluate all the three stages of the peri-operative care.
More often a nursing assessment is based on the medical side of the patient rather than the holistic approach. In this assignment I will be discussing the importance of the nursing process, care planning, and looking at how these are used in practice. I will look at the tools used in the nursing process and show an understanding of how effective they are when used correctly. I will achieve this by describing a case study of a patient from my practice area, and discussing two specific areas that affect the patients care. Throughout this assignment I will be using a pseudonym to maintain patient confidentiality in order to conform to ‘The Nursing and Midwifery Code’ (NMC, 2008).
Page 3 Quality patient care can be advanced throughout the hospital by providing nurses with data such as the number of incidence of pressure ulcers and ways to prevent this from occurring, by providing education on restraints and when and when not to use them and how to use them correctly. Examining data outcomes can improve the quality of care by exploring the improvements that have been made through implemented changes. All in all, good patient care is common sense. Listen to the patient when they talk, they will tell you everything that they know, it is up to you to fill in the blanks. Look on the internet after your admit and research a person’s culture, it can prevent problems and embarrassment later as well as keeping the patient comfortable and safe.
Throughout their lives this group will require specialised tests and examinations to prevent damage to the bladder and kidneys. Methods of voiding and medications will be discussed with the client, continence nurse and other agencies to provide a package of care. As spina bifida is considered a neurological condition it is covered by the National Service Framework relating to long term conditions. The NSF has divided neurological conditions into four categories and spina bifida generally fits into the ‘stable but with changing needs’. The Framework is to be used by both clients and professionals and identifies targets for both health and social care service providers to assist clients in their daily living requirements.
Describe the elements of a comprehensive health assessment of a geriatric patient. What special considerations should the nurse keep in mind while performing this assessment? Nurses and healthcare professionals need to pay close attention to different things while performing a comprehensive assessment on the older population. The comprehensive assessment includes mental and functional status, social and economic status and the actual assessment of the body functions (Jarvis, 2012). It’s important to observe mental status changes and functional status changes, this can determine how well the patient can take care of themselves and deal with their health promotion on their own.
