The diagnoses must be based on the case study, be appropriate, be prioritized, and be formatted correctly. For each nursing diagnosis, state two desired outcomes using NOC criteria. Desired outcomes must be patient-centered and measurable within an identified timeframe. For each outcome, state two nursing interventions using NIC criteria as well as one evaluation method. Interventions and the evaluation method must be appropriate to the desired outcomes.
Determine three patient safety issues that are being addressed by your health care organization. Identify the actions the organization is taking or identify three issues that you think need to be addressed by health care organizations. Make suggestion for actions to be taken. Support your responses with two peer-reviewed references. HLT 308V Week 4 DQ 2 The Occupational Safety and Health Administration (OSHA), the Centers for Medicaid and Medicare Services (CMS), and The Joint Commission (TJC) have identified the necessity for health care organizations to have a risk management program to address infection control.
Scenario A female is referred to home health services for skilled nursing evaluation, and observation. The patient was discharged from the hospital a few weeks ago diagnosed with a Cerebrovascular Accident (CVA). The skilled nurse was recommended to teach, train, and monitor the effectiveness of new prescribed Coumadin therapy. Standardized Terminology Application The registered nurse selected the appropriate clinical diagnoses using the North American Nursing Diagnosis Association (NANDA) terminology based on patient’s actual needs, and comprehensive assessment. The care plan was created using a linkage between diagnoses, interventions and patients’ desirable goals.
11/12/2012 Components of a Nursing Care Plan A nursing care plan is a set of guidelines that directs nursing activities for a patient. It details the entire steps needed from beginning to end of a patient’s care. The nursing care plan is also a framework that allows for evaluation of the effectiveness of the plan and it promotes continuity after the patient has left the healthcare facility. The four main components of a nursing care plan are: nursing diagnosis; expected outcomes; interventions; and evaluations. Documentation of the nursing care plan is important in order for the patient to receive proper care.

The people involved in the RCA should be the people involved in the scenario: the RN (Nurse J), the LPN, the physician (Dr. T.), the emergency room manager, and a figure from administration (Chief Nursing Officer?). These participants should conduct a RCA to determine the causative factors that lead to Mr. B’s sentinel event. The first step would be to gather data about the situation. Mr. B’s presentation, vitals, health history, lab values, pain score, medications he already takes, and medications he received (amount, dose, and times) during the conscious
This paper will address the foundational frameworks of QI, the various stakeholders’ definition of quality, the various roles of clinicians and patients in QI. This paper will also address why quality management is needed in health care industry, accrediting and regulatory organizations involved in QI. The Foundational Frameworks of QI The foundational framework of QI is a continuous process that focuses on multiple relationships such as implementing improvements and improvements in processes. Some areas that organizations may concentrate their improvement efforts on are the reduction of medication errors, reduction of emergency room wait times or clinical measures such as breast cancer screenings or HIV testing. Walter Shewhart developed the Plan, Do, Study Act cycle used as the basis for planning and direction performance improvement efforts (Ransom, Joshi, Nash, & Ransom, 2008).
The risk assessment plan should involve regular skin inspection of all patients who are bedridden for long periods (Hoeman, 2008; Carpenito-Moyet, 2009). Different risk assessment scales will be used to determine the patients who are at a higher risk of pressure ulcers. The Modified Norton Scale will be used to assess the mental state, mobility, physical state, and general health of patients. A score of 31 and below will indicate the possibility of developing pressure ulcers. The Braden Scale will be used to evaluate the sensory perception, friction and shearing.
* What do you think should be competency requirements for basic peripheral IV skills? What are the competency requirements for basic peripheral IV skills? * Infusion Nursing * Infusion nursing is defined as the nursing process relating to technical and clinical application of fluids, electrolytes, infection control, oncology pediatrics, pharmacology, quality assurance, technology and clinical application, parenteral nutrition, and transfusion therapy * Three-Part Competency Model * Competency statement * Statement that reflects a measurable goal * Domains of learning criteria * Cognitive (knowledge based) * Performance criteria (psychomotor
The methodology used will be described and the rationale for this choice. The recommended sampling, data collection and analysis will be discussed and an explanation of the approaches adopted. Trustworthiness and rigour of the study and ethical issues and considerations will be referred to within context of the proposal. Finally it will be concluded as to how the proposed piece of research would add to the knowledge of nurses and the benefit it would have on their practice as well as discussing the impact it could have on patient safety and the quality improvement of care. Literature review A literature review is undertaking to summarise all current literature on a selected topic.
