The risk assessment plan should involve regular skin inspection of all patients who are bedridden for long periods (Hoeman, 2008; Carpenito-Moyet, 2009). Different risk assessment scales will be used to determine the patients who are at a higher risk of pressure ulcers. The Modified Norton Scale will be used to assess the mental state, mobility, physical state, and general health of patients. A score of 31 and below will indicate the possibility of developing pressure ulcers. The Braden Scale will be used to evaluate the sensory perception, friction and shearing.
Week 5 DQ1 The elements that should be included in a comprehensive health assessment for a geriatric patient are the clients past medical history, family history, comorbid conditions and severity as well as current problem list. Current medications should be reviewed in detail, as well as a head to toe physical of all systems. (Ward & Rubin, 2013) The physical exam is an excellent opportunity to check the aging clients hearing and vision. It is also an opportunity to do a cognitive assessment, and assess for bruises or injuries that could indicate an unsteady gait or elder abuse. The clients weight and appearance are an indicator of nutritional status, loose fitting clothing could indicate recent or sudden weight loss, eating habits and dental issues should be assessed, as well as who is preparing the meals.

Nursing Assessment Deakin District Nursing Service visited Mrs Janet Smith on June 16th2008; this was to undertake the first nursing assessment within the client’s home environment. Throughout this visit with Mrs Smith, it was noted that she lives with her partner Mike and their children. Currently the children are staying with grandparents whilst Mrs Smith is unwell. Mrs Smiths partner states he does not believe Janet requires home nursing and would like to be left alone to manage. It is also noted the family home which is situated in Melbourne’s Outer East, the garden is poorly kept with cracks and holes in the concrete path leading to the front door.
I feel that withholding information on a lucid patient fully in control of her mental faculties is tantamount to lying. I believe the patient has the right to know the conditions surrounding her health care treatment plan. Step 4 I verbalize the problem by saying, “Should I inform the patient of her terminal health care condition or should I follow the wishes of the patients’ daughter and withhold information on the seriousness of her condition?” Step 5 There are many courses of action possible, but I believe the best outcomes would like along these three possible scenarios. The first option would be to disclose all medically pertinent information to the patient so that she is fully informed. The second option would be to as for assistance from a social worker or a preacher or priest from the members church to talk with the daughter and mother.
As a health care professional trained in different approach, I assessed my client based on the theory and learning experience that I have had. The client was been diagnosed with dementia, limited mobility, and inadequate verbal communication. I undertook a full assessment to a client with a sacral pressure sore. Assessment using observation was been completed to the sacral area, and graded the level of pressure ulcer using the Braden scale. The nurse mentor was been informed about the type of dressing and intervention that should be provided to the client, along with the explanation with the rationale to the procedure that I have decided to use.
