Assignment 9 1A) There is no doubt that person-centred values must influence all aspects of health and social care work. Health and social care should base on person-centred values, and should be individualised because this is law requirement (see Human Rights Act 1998, Health and Social care Act, Code of Practice for Social Care Workers, etc.). If person-centred values become guidelines in health and social care setting it will definitely help to provide anti-discriminatory practice and help to promote and support individual’s right to equal opportunities, dignity, independence, choice and safety. As health care workers we need to respect people’s rights, beliefs and identities, and we also need to enable them to make or participate in making decisions relating to their care, treatment and lifestyle. The key areas of support that should be available to the person are: * Practical/physical support * Emotional support * Social support * Intellectual/cognitive support 1B) Care plans are the basis for how social care and support services are delivered.
Working alongside the guidelines of person-centred values should eliminate discrimination, abuse, lack of dignity and respect, service users making their own choices and restricting individuals being an individual. Understand how to implement a person-centred approach in an adult social care setting. 2.1 Finding out the history, preferences, wishes and needs of an individual contributes to their care plan in many ways because you can learn a lot from a person, finding out their likes and dislikes by knowing this information you can promote their choices. Knowing the history of an individual will help offer support in the most appropriate ways to that individual.
* Everyone has a unique history and ‘life story’. This will affect their needs, life choices, and their behaviour and it can only be affective if it takes the unique life history into account. * Person centred care and personalised approaches have very positive outcomes for service users, it has been reported that improvements in general health and well-being. * Working with a person-centred approach means you can adapt your approach when situations and/or needs change. * Everyone wants to have control over their own life and has the right to make choices-
An advocate is a person who intercedes on behalf of another person in a bid to ensure that their best interests are communicated. Involvement is the inclusion or engagement of the patient and can relate to communicating with patients regarding their planned care. Participation takes involvement a step further, and relates to taking a more active role and sharing. This can relate to the health and social care provider actively engaging the person in the development of their care plan. Trust relates to having confidence in another person - that is, to having faith that they are reliable and honest.
Centrality means looking at a particular individual as a whole, taking into consideration their wishes, feelings and aspirations, and not only focussing on their disability. You need to work in a person centred way which means putting the individual at the centre of everything you do for them. The individual need to be involved in their care planning and all decision made. Their wellbeing and self-esteem taken into account. By recognising the centrality of the individual, we empower them and boost their confidence.

Not only will the diversity of our learners be valued but we must promote equality in all aspects of our role as a teacher or facilitator. We must understand the boundaries set to allow us to refer learners to other professionals were there is a need and know to whom they should be referred when the need arises. . It is vitally important that we, as teachers maintain and develop a safe and nurturing environment in which our learners can, not only learn new skills but develop appropriate behaviour and respect for others. Reece and Walker (2000 p5) says that “A programme where the contents is carefully derived from an analysis of the student’s personal, social and /or vocational needs ,and which is implemented by you in such a controlled and organised manner that the student is almost certain to learn, and is aware when the learning has taken place.” 1.1 As a company delivering both construction training and foundation learning we take the “Health and Safety at Work Act “(1974) into consideration every day.
I need to give and receive important information and instructions. I need to communicate to help people understand, to be understood, to deal with individuals emotions and feelings, to give encouragement and to show others that there are valued. Communication is an essential part and a basic requirement of being a support worker. Effective communication also ensures that all health and safety issues are spotted and reported. Effective communication from others will ensure their preferences and needs are met.
1.1 Describe how accessing a range of services and facilities can be beneficial to an individual’s well being. Regardless of the disability a person should be able to access care services in the community because it is important to their wellbeing and health. It also gives the individual more independent. These are the services and facilities that impact directly on the quality of everyday life of many people with disabilities, their families and carers. There are many services and facilities which the client can access which will be beneficial to his or her well-being.
Communication is essential and is used to meet needs of all children. It is a basic requirement of my job role to communicate with individuals and their families, other members of staff on a daily basis. Explain why it is important to observe an individual’s reactions when communicating with them. Observing individuals helps us learn about their needs, how their feeling, what they lie/dislike, what they enjoy doing in certain activities. It is important to do so because it ensures any health and safety issues are recognised and reported, for example any illnesses, disabilities ECT… Be able to meet the communication and language needs, wishes and preferences of individuals.
