Promoting Interdisciplinary Care As the nursing supervisor I would promote using a team approach to improve quality and continuity of care for patients. An interdisciplinary team would involve layering of medically trained professionals from various levels of education and talent. This diversity would allow every aspect of a clients’ needs to be adequately met. In this situation there is a potential group of similar patients (although we focus on one). The first step I would take would be to introduce each team member and educate each member on other their role and the role of others. Delegation/Teamwork, Responsibility, & Recommended Resource As not only the nursing supervisor, but also a member of the the interdisciplinary team I would approach the FNP, Ms. W in a calm and private manner. While talking with Ms. W I would explain that she should not feel like she is solely responsible for Ms. R’s complicated case. I would suggest that Ms. R’s special circumstances be introduced and discussed thoroughly with the entire interdisciplinary team to reach a favorable outcome. Although in this approach all team members are equals, Ms. W (the most educated) is still responsible for delegating and using all of her resources. As the “most medically trained,” Ms. W plays a significant role as a member of the interdisciplinary team. Not only should she assess and provide care to the client but she needs to appropriately delegate to the other medical professionals at the clinic. Delegating involves assigning tasks, both physical (assessment) and mental (teaching) that are within the team members’ scope of practice. Ms. W must at this point, be educated and able to recall the “roles,” or legal definition of each members’ job title. After appropriately delegating, Ms. W must then supervise that all duties are done in the right manner and within scope. R, the proper use of each and every team member (delegated to appropriately) makes for a potentially good outcome. The clinic’s nutritionist could offer dietary advice that may help not only maintain a healthy pregnancy but aide in the recovery from her chronic UTI’s. The nutritionist may suggest a simple and healthy dietary regimen along with cranberry juice supplemented to help Ms. R’s urinary tract’s acid base balance recover (from UTIs). The LVN, can educate Ms. R on the remaining prenatal care needed, labor/delivery, parenting and infant care. Since there is a language/cultural barrier involved with Ms. R’s case the Latina social worker is key for proper communication. The social worker can help translate between care givers and clients plus their family. Lastly, the community health nurse can assist the team in the use of proper community outreach and resources. Such resources for Ms. R may consist of options such as parenting classes and welfare assisted daycare organizations. As a whole the interdisciplinary team will work together to provide Ms. R with educational and physical wellness for herself and her unborn
