You are upholding someone’s dignity by recognising what is important to them by making sure they are aware of the risks before they carry out an activity. How does using an individuals care plan contribute to working in a person centred way? The customer you are caring for has been an active participant in creating their care plan by recognising what they can do for themselves.

This is based around each individual and their careplan so the care i give is individual to them. I assist with personal hygiene, toileting and administer medication. As much as i enjoy assisting clients i always encourage them to be as independent as possible by letting them plan their day and making own choices. Also providing the clients with a welcoming and friendly environment will make all the difference to the person settling in. This provides stability for them and a sense of belonging.
We are all individual, no two people are the same hence it is not appropriate to say that because two people have dementia that they both have the same care and support needs. When I am carrying out all these, I do remember person centred values and this helps me to be able to do what is right for the client or what the client wants. Before carrying any activities for the day, I do go through my clients care plan; follow the team meetings to get all the information about the clients and the changes. For example if I have arranged with my client that he or she had to have a shower, then when the time comes they change their mind, it is for me to agree with them because it’s their right to change their mind. At this stage we communicate together and sort something else for another day. Personal relationships are based on emotions and are informal.
