Primary care nursing is a standard practice in the intensive care unit, and by nature is a proponent of transformation. Patients benefit from the evolution of nurses obtaining higher levels of training and education. For example, a nurse in the intensive care unit through vigilance and training can affect incidence of ventilator acquired pneumonia and central venous catheter infections. Hospital induced delirium is a very expensive add-on to the cost of healthcare, but through early detection and treatment can be minimized by the bedside nurse. Essentially, if nursing performs at the peak of their training and education we can greatly impact in every area the goals of the IOM report.

Nursing theories provide structure to the critical thinking and decision- making processes. Theory, research and applications to clinical practice are symbiotic as new concepts, practices and empirical data emerge professional practice changes or evolves. Florence Nightingale’s writings originating in 1859 are considered the first nursing theories postulated. She is known for decreasing mortality rates in hospitals during the Crimean War (George, 2011).
The ability of nurses to respond, preform, and resolve issues accordingly in nursing care situations can be attributed to the use of theory. Nursing theory propounds enhanced problem solving abilities for those who know and practice it with the result of better decision making that is organized and purposeful (Abdellah, 2014). According to Abdellah (2014), in order to execute quality nursing care, the ability to identify and solve overt and covert nursing issues is fundamental. Application of a theory to address and resolve challenges within the nursing profession can be employed through health policy, institutional and operational dilemmas, and within the management of nursing care services. There are several disciplines that comprises a theory which includes middle range, grand, or practice nursing theories.

As one explores the history of nursing, it is amazing to see the vast contributions nurse leaders have made to everyday patient care. Provision nine states that a nurse is responsible for articulating nursing values and maintaining integrity of the profession (Martha D.M. Fowler, 2010). This can be accomplished by becoming a mentor for nurses who have just graduated. I could sign up to be a preceptor and really share with the new nurse the values of nursing, as well as demonstrate the integrity and core values of the profession. I could easily go through each of the nine provisions to the ANA Code of Ethics and demonstrate how I can use each one in my daily life as a registered nurse.

While being precepted with other nurses in this field, Jill learned very quickly that Dr. Cook was not a subtle or friendly Doctor to work beside, but someone who was very particular and upfront when stating what she wanted. Dr. Cook was more of a “in your face” kind of woman. Even though Jill was told to avoid Dr. Cook at all costs, she fell into a serious issue. As Cook was in the middle of a procedure, Jill noticed Dr. Cook had touched the overhead light with her sterile gloves which were now contaminated.
