Person Centered planning is a way of enabling people to think about what they want now and in the future. It is about supporting people to plan their lives, work towards their goals and get the right support. Person Centered Planning is a collection of tools and approaches that can be used to plan with a person- not for them. Planning should build upon the person’s circle of support and involve all the people who are important in that person’s life. Person Centered planning is built upon the values of inclusion and looks at what support a person needs to be included and involved in their community.
CU2654 Provide Active Support 1.1 Explain how the key characteristics of active support differ from the hotel model The hotel model is where the staff take over and the service user is a spectator of their own lives however active support encourages the service user to take active participation in every part of their lives; it provides the support that is needed to meet the needs of the service users. Active support is a way of making sure service users are able to engage and participate in their own support. 1.2 Define the terms Promoting independence: Means when service users have the power to decide different options available to them by providing them with a variety of information, they can own decisions, and share responsibility for the outcomes. The purpose of this is to help service users to regain or learn new skills and to live their everyday life and stay within their community Informed choice: Means that about service users’ rights for a decision about diagnostic or therapeutic procedure that is based on choice, which requires that the decisions be voluntary and that the client has the capacity for choice. This rest on 3 elements: possession of a set of values and goals, ability to understand information and communicate decisions, and the ability to reason and deliberate.

It means thinking about the person in a holistic way - so their previous history, spiritual needs, physical needs and family are considered - likes and dislikes. They are encouraged to get involved in activities within the home and promote or maintain their independence. The hotel model on the other hand, is just a 'new' word for institutional care - things done without real consent and for the convenience of the staff (like making people wait for pain relief because the medicine round isn't officially starting for another hour). The hotel model means that people have everything done for them which means they are losing their skills and ability to care for themselves.
Reflective practice is essential to personal development and the goals and targets put in place at performance reviews can help to focus you and give you a renewed sense of ability to succeed and achieve. Effective written and verbal communication and being able to use a variety of interpersonal techniques promotes strong working relationships which will then lead to a good quality care for my service users. With service users I initially need to establish a service users ability, needs and preferences in relation to communication needs and develop a plan of action to provide appropriate support that meets the service users communication needs and methods. The assessment involves observation questions and regarding information on a number of areas including ways and their preference of communication by service users which support process of completing the service users person centred plans. Any changes to service users’ needs need to be recognized and care plans changed to support the changes. When conversing with service users I ensure I speak in an informal relaxed way I
