Also, I highly agree with Peplau’s theory, this theory emphasizes the importance of patient and nurse interpersonal relationship and determines its importance for caring patient (Potter et al., 2011, p. 44). This is an advantage because people are more comfortable in surroundings they are familiar with. Hospice sends nurses out to the person’s home periodically, and can be reached at all times. If there is a need for assistance with personal care, the hospice will send out a certified aide. The doctor of the person receiving care can give orders to the hospice for pain management medications. The nurse can administer the medications to the patient as needed.
We will have temporary housing for them as we seek to find more permanent housing and connect them with resources such as, insurance, SSID, SSI, medical care, rehabilitation, counseling, education and jobs. SECOND CHANCE will have a nurse and a social worker to help them get clean and connect them with the resources they will need to continue to stay clean and get any medical needs taken care of, along with helping them connect to any other resources they might need. We want to help them improve their quality of life and know that having a house first will help them start to make the necessary steps to move forward in life. The student nurse hopes with this assignment to allow the reader an insight into my experience during my management placement within a busy rehabilitation ward in Wishaw General. With the guidance and supervision of my mentor I participated in all areas of running a busy rehab ward. The student nurse will reflect on health promotion issues as this stood out to me in several areas, The student nurse discovered that although we can nurse a patient back to health it is so important to make the patient aware of how necessary it is for them to participate in their care by adhering to what is recommended to them by various members of the Multidisciplinary Team and the devastating consequences that the patient can encounter if advice is ignored.
During the assessment phase the nurse will continuously gather data about her patient. For example taking vital signs, observing breathing patterns and monitoring blood glucose will give us internal details about how the patient is doing.
