Introduction Organization’s Goals * To provide housing to individuals that are both homeless and disabled. * To assist them with social services, such as benefits, permanent housing, jobs, medical needs and rehabilitation. * To empower the residents and give them the resources to become productive members of society. Philosophy To provide housing first to homeless individuals with disabilities and assist them in improving their quality of life. We will have temporary housing for them as we seek to find more permanent housing and connect them with resources such as, insurance, SSID, SSI, medical care, rehabilitation, counseling, education and jobs. SECOND CHANCE will have a nurse and a social worker to help them get clean and connect them with the resources they will need to continue to stay clean and get any medical needs taken care of, along with helping them connect to any other resources they might need. We want to help them improve their quality of life and know that having a house first will help them start to make the necessary steps to move forward in life. Social and Economic Costs Homeless costs the community and the taxpayers a lot of money. If we could effectively deal with the situation over time then you are going to save money by fewer individuals utilizing services that are costly to the taxpayers. There is also Projects for Assistance in Transition from Homelessness and the American Recovery and Reinvestment Act, however there is still not enough resources to provide adequate services to the homeless population. There is also the Supportive Housing Program that has started in the state, but has not made it into our area as of yet. A Plan of Action * This is a fifteen bedroom house that will have offices for the nurse and social worker. * We will provide a place for people that are both homeless and disabled and help them receive the resources they need to live on their own. * The nurse will be responsible for all medical care, such as withdrawal from drugs, administrating appropriate medications, and any other medical care that would be needed. * The social worker will be responsible for connecting the residents with the resources they will need. This includes finding permanent housing, getting health insurance, finding a job or education training for a job, rehabilitation, drug and alcohol treatment, VA benefits if a veteran, mental health screening, and any other social services that may be
