Durkee (2007) advises the implementation of a medication reconciliation “work group” to facilitate the process. A work group underscores the multidisciplinary nature of healthcare in general, acknowledges the whole patient, and also shows how specialized a function medication reconciliation can be. Typically, “there are at least three disciplines generally involved in the process—medicine, pharmacy, and nursing—with little agreement on each profession’s role and responsibility for the reconciliation process,” (Barnsteiner, 2008).
