An interview was taken from a 28 years old female health care professional from Pakistan. She was born and raised in Karachi a city of Pakistan, and has moved to US three years ago with her husband who is also a physician by profession. She completed her medical schooling in Pakistan and worked as a physician for three years as a gynecologist especially in rural areas of Pakistan. In US she has worked as an intern in a private hospital in Houston (TX) for nearly a year. I interviewed her with the consideration that she is the best person to describe the health system both of US and Pakistan as she was a part of this system and has closely observed the health care delivery in both the countries. The worse thing that I came to know from interview is these people in remote areas tend to follow the non qualified health care providers at the local level that are called Hakims by the local people. These persons may be dangerous and need to be controlled on serious basis as the quality of the services provided by them is not up to mark and they are performing illegal function that cannot be relied upon. When asked about the awareness of people about health she seemed to upset about the level of knowledge in the rural people of Pakistan. There are certain programs run by the Government that train some of the local people as health care workers and after being trained they can go back to their villages and can make the villagers aware about drinking clean water, good sanitation and immunization. The most disturbing thing she told about the level of education in villages is the application of Ghee; a clarified butter used for cooking, applied to neonate umbilical wounds and is prepared using dried cow dung as fuel that is a major cause of neonatal tetanus in the country.
