Short answer questions Explain what is meant by the term ‘active participation’ This means involving with the individual, interactive with service users for example supporting or encouraging with games, dressing, feeding and medication. Explain how active participation can support all aspects of an individual’s preferences and needs. Explain how to ensure everyone involved in an activity or event such as a review understands how to apply the principles of active participation. As carers we need to understand and respect that every resident has their own individual needs and requirements and we need to show support and understanding for these wishes. . We should have discussed with the resident and offered choices of drink and food which they are allowed to consume within their religion. It is important that these choices are recorded in the residents care plans and everyone who is providing care or working with the best interest for the resident are fully a ware of the choices the resident is choosing to make, and any new members of staff have been informed of the residents needs and wishes. 2.When a resident is making unhealthy choices for their condition There are many different types of allergies
Communicating with other staff members ensures effective team working and continuity of care. It also ensures any health and safety issues are recognised and reported. All carers record notes about their visit in log books after attending an individual, thereby keeping other staff informed and aware of current situations with the individual. This is based around each individual and their careplan so the care i give is individual to them. I assist with personal hygiene, toileting and administer medication. As much as i enjoy assisting clients i always encourage them to be as independent as possible by letting them plan their day and making own choices. Also providing the clients with a welcoming and friendly environment will make all the difference to the person settling in. This provides stability for them and a sense of belonging.

I assist patients at meal times attempting to promote a healthy diet. I assist qualified staff by carrying out a range of tasks e.g., taking physical observations: BP, Pulse, completing safety checks and observations. I have a reasonable understanding of the Mental Health Act and am able to file important documentation in patient’s notes, so that they are easily accessible whilst maintaining confidentiality. I have developed a wide range of skills while working for the health board. I have learned how to build relationships with fellow staff to work as part of a team.

Working in a partnership with them in developing and assessing their needs to suit their health care thats needed, their wishes and choices and dignity respected. Also in promoting their independance and giving them their choices and control in their lives and working with them as a team alongside other care proffessionals and family members but respecting their privacy at all times. Effective written and verbal communication and being able to use a variety of interpersonal techniques promotes strong working relationships and will promote good quality care. In my role it is important that communicate in the correct way to ensure that the needs of the service users are understood and dealt with correctly. On an initial assessment with the service user we discuss their needs and wishes to ensure they receive the right service especially made for them. I will continue to have regular meetings with them to establish their changing needs. My Manager, myself and all the care team have regular meetings to discuss all the service users and if any of their needs have changed and to be sure that everyone knows what is happening, if any changes have been made.
