M2 – describe how three key professionals could be involved in planning support individuals. For this task I am going to pick three key professionals and then describe how they are involved in planning support for individuals. I will include a case study and then how the three professionals are involved in the support process. The three key professionals I have chosen are: * GP * Social worker and * District nurse GP (General Practitioner) The individuals GP would be a main professional involved because they are their main doctor and would be the first person to know about any illness or support that individual needs. A GP would be a t the local doctors and would be where the individuals go to have check ups or when they have a cold and they would go to their GP for a treatment. Many individuals would have been going to their GP for many years which means the GP knows the individual very well, their living conditions or previous illnesses that may need to be brought up whilst planning support. Their GP would also be in contact with the other professionals to inform them of any background they need to know, what medication the individual is on (if they are on medication) and why they are on that medication. Because her mobility was decreasing and Eileen was in some amounts of pain with her hips, the nurse prescribed her with some very low level pain medication just to ease of the pain. She also wanted Eileen to do certain exercises daily to loosen up her muscles a little bit to ease her walking.
