Noncompliance, due to the several steps and time pressure, and variation in the manual method is believed to be at fault. It was decided to trial the use of SwabKit® in early 2010 on the critical care units, medical-surgical units, and step down units. They used the caps on central venous catheters (CVCs), peripherally inserted central catheters (PICCs), and peripheral intravenous (PIVs). The trial was successful, so they decided to implement use of the kits hospital wide in July 2011. After a two month adjustment period, the hospital began to record data on the use of SwabKit®.
May is a slight exception where nursing care hours/pt day was over ten and there were still 7.9 falls recorded that month. Other than that where the falls occurred less is when the average nursing care hr/pt day was less also. This speaks to the fact that hours of care is not necessarily the factor that will reduce falls. As noted in the Joint Commission Perspective article referenced previously skill mix and competency of staff is a factor. Experienced nurses, nurses with additional and continued training, and a plan for preventing falls on the units is what will be required for Nightingale to reduce patient falls on our units as well as be in compliance with the Joint

Executive Summary – Middlefield is facing the high employee turnover, workforce shortage - especially of Nurses and low employee morale problems. Some of the findings about the causes of the problems are opening of the new hospital with better facilities and advanced technology for patient treatment and care, unavailability of quality instructors for nursing degree programs at universities. To tackle the problems, efficient use of existing Nursing workforce should be done in the short term. Whereas aggressive retention policies and increase in production of quality Nursing workforce should be long term strategies. Also Middlefield must ensure to increase the employee morale.
State the null hypothesis for the Baird and Sands (2004) study that focuses on the effect of the GI with PMR treatment on patients’ mobility level. Should the null hypothesis be rejected for the difference between the two groups in change in mobility scores over 12 weeks? Provide a rationale for your answer. The null hypothesis is that the woman receiving guided imagery show no greater improvement in their pain scores than the control group after 12 weeks. The study results indicated significant improvement in mobility scores of women who received treatment (F(1, 22)= 9.619, p = 0.005).

While studying the surgical patient tracer worksheet, one of the most serious deficiencies identified was the patient history and physical not being done within twenty-four hours of admission. In fact, the patient medical records were completed after more than seventy-two hours of patient admission. Documenting medical records in an appropriate time frame is an important standard in the joint commission accreditation process. The Joint Commission requires an accredited hospital to have written policies regarding timely documentation into medical records. Eighty percent of a patient’s diagnosis is done by the identification of their current and past medical histories.
Patient's are the main/only ones we should focus on when implementing an EHR because as the name states, it is their electronic health RECORD. FALSE 3. When starting process mapping for an EHR implementation project, it is important to include all processes in a facility, not just those that will be touched by the EHR project. FALSE 4. At Hospital XYZ, during the morning period of 8am to noon, the wait time from when a physician orders an X-ray to the time the patient returns from having it done is too long as this is when most of the X-Ray orders are given and there is a backlog of patients.

Then, her Doctor, and her power of attorney were contacted due to the refusal of medication.
