Acute Cystitis

Introduction: Marvin Brown, 42 year old male complains of having to void constantly, as well as painful urination and hematuria. He noted that immediately after he voids he feels the need to void again but nothing comes out. He states that this has been going on for about a month, but refuses to visit the physician because he feels it’s a waste of time and money. He’s upset because majority of his day is spent standing over the toilet. After experiencing low back pain he decided to make a visit. The focus of nursing care is to monitor lab values, identify ways to promote healing, or eliminate discomfort.
History:
Marvin was diagnosed with Benign Prostatic Hyperplasia one year ago. He’s non-compliant with the treatment regimen and drug therapy and hasn’t followed up with his physician for further evaluation.
Pathophysiology:
Marvin has been diagnosed with Acute Cystitis. Cystitis is an inflammation of the urinary bladder. Inflammation is caused by bacterial infection. (Lippincott Williams & Wilkins, 2010, p.945) A common cause of Cystitis is Benign Prostatic Hyperplasia. (Lippincott Williams & Wilkins, 2010 p.946)
Nursing Physical Assessment: Marvin is A&O x3. He can ambulate without assistance. Vitals are; B/P 120/82, Temperature 96.9F oral, Apical Pulse rate 81, R 20, SP02 99%. Bowel sounds active. Head to toe assessment show skin intact. Lab values; RBC’s is 6.3; normal findings are 4.7-6.1. WBC’s is 13,000. Normal findings are 5,000-10,000.
Intervention:
The doctor has ordered a microscopic exam of the urine which reveals an increase in the number of red and white blood cells, as well as a culture and sensitivity studies are used to identify the causative microorganism and appropriate antimicrobial therapy. The physician prescribed Marvin trimethoprim-sulfamethoxazole (TMP-SMZ), and nitrofurantoin macrocrystals (Macrodantin) as
