We have been looking after JM for a number of years she came to live with us with early stage dementia as her husband BM could no longer care for her himself at home as he was suffering from heart disease and diabetes but he still regularly visited her as he lived a short distance away. Whilst in our care her dementia has deteriorated slowly but she would still recognise her husband and family when they came to visit. As a benefit of working at our hospital, we can use our cardiac rehab gym for our own use. As I would do my daily workout, this sweet little gray haired lady befriend me and as we got to know each other, I discovered she had just in the last six months had a triple by-pass, still has a mild congestive heart failure and was in the midst of leukemia/lymphoma diagnosis. Reflective account on: Be able to monitor care plans Q 5 Mrs S lived alone and had carers going in to assist her 3 times a day. Her health started to deteriorate and was having lots of falls when no one was there. Neighbours would sometimes go in to help her but her health continued to worsen. Her care package was increased to the maximum so she could have check calls throughout the day. It did not however stop her being taken to hospital at least once a month because of other problems she had as well as the falls.
