OLDER ADULT Beverly J Sinclair Franklin Pierce College ABSTRACT K.J is a 76-year-old white male that was admitted to a health and rehab nursing home for strengthening. K.J is planning to go home with continuous oxygen. K.J is a 75-year-old white male admitted to Rowan court Health and Rehabilitation from Central Vermont Hospital. Resident’s reason for seeking health care was for Rt knee pain. KJ states that he has had right knee pain for about a month. His pain gets excruciating during the night. He has a diagnosis of pseudo gout. He states he has never seen an orthopedist for this and he does have a history of DJD. HEALTH PERCEPTION-HEALTH MANAGEMNET…show more content…
Aspirin 81mg daily 4. Lasix 40mg daily 5. Omprazole 20 mg daily 6. Pravastatin 10 mg every evening 7. Vitamin B12 daily 8. Folate 1mg daily 9. Nitroglycerin 0.4mg sublingual as needed. No known Drug Allergies noted. K.J’s perception of his health is fair. He does not exercise on a regular basis. Resident follows prescribed regimen. While resident is at rehab he uses 2 ½ side rails for mobility and independence. NUTRITIONAL-METABOLIC K.J’s weight is currently 212 and his height is 6 ft .He is on a regular lo salt diet. His appetite is normal. Resident feeds self. Condition of mouth is pink. No ulcerations or lesions noted. Resident has edema bilateral Lower extremities. +3 pitting in the Rt lower extremity and +1 in the Left lower extremity. K.J is currently wearing TED stocking during the day. Client’s body temperature is WNL tympanic. ELIMINATION Medium formed Dark brown stool. Last BM was Today. No reports of constipation or diarrhea or incontinence. Resident states that he urinates frequently. He states that he voids about 10 times a day Partly due to medication. Abdomen is soft and non-tender. Good hygiene. No anxiety noted. Residents talks and answers questions appropriately and readily. View of self is positive. ROLE-REALTIONSHIP KJ lives alone, has never been married or in any relationship. KJ denies having any children. KJ had a brother which is now deceased. His only living relatives are two nieces and two nephews. He is retired and has been for many years. He worked as a dairyman for many years. KJ’s support system is his nephew and niece. His niece works as assistant director of nursing at this facility. KJ has concerns about the changes in his life. He is concerned about getting water. Which he stated he currently goes to the Lander mat two-three times a week and carries 8 gallons of water home for washing dishes and bathing. Another concern he has is not being able to do this. Mainly because he will have to continue with oxygen. SEXUALITY-REPRODUCTIVE KJ has a history of BPH. No other concerns noted in this area. COPING-STRESS TOLERANCE KJ’s main concern is being able to go home and be able to maintain his life. The resident understands that he will have to continue with oxygen all the times. VALUE-BELIEF KJ’S religion he practices is protestant. He is not an avid churchgoer; instead he reads the bible
