Serious Failings In Care- Daniel Pelka Trial The Daniel Pelka trial highlighted the horrific abuse that the 4 year old boy suffered at the hands of his Mother and her abusive partner. However it also displayed a clear lack of attentiveness and communication between the people who could of taken responsibility for the childs wellbeing. Daniel Pelka was found dead in 2012, and the circumstances surrounding his death were horrendous by nature. The cause of death was found to be primarily starvation, although the child displayed clear signs of long term abuse, and was described by medical professionals as 'resembling a concentration camp victim', due to the severity of his abuse and weighing over a stone below the average expected weight of a child his age. His Mother, Magdalena Luczak and her partner, Mariusz Krezolek were subsequently tried and convicted of a series of abuse charges, which included imprisoning the boy for several years and force feeding him salt.
To identify that should be made, each services involve do an individual management review of its practises. Local authorities should inform the Ofsted of all serious incidents involving a child that can lead to a serious case review including where the child as died or may have suffered significant harm as a result of abuse or neglect. Serious case reviews include the importance of: Sharing information and communication Keeping an accurate timeline of events Clear planning and role Overcoming the problems of hard to reach families Good assessment of the child’s situation Early recognition of the child’s in need of protection by mainstream services such as schools or health services Inquiries and serious case review has been in place when practitioners and other agencies failed to identify that Victoria Climbie has been abuse and she had died of it. Lack of mental health services is a huge reason. In November of 2013, a Virginia state senator, Creigh Deeds, was stabbed by his son, Gus, who was mentally ill. A day before this occurred; Gus was not emitted to a psychiatric hospital because there were no beds available (Paul Schwartzman Page#1). Those mentally ill who cannot get the help needed due to money issues, or not having enough mental health services reaching out to them, might lead them to commit suicide, kill or harm others, or end up in jails and prisons.
