Assessment and Care Planning for the Adult in Hospital Care planning is the most important aspect of holistic care even though it is highly underestimated and often neglected. A holistic assessment involves looking at the individual as a whole rather than their illness. By doing this we can form a balance between the nature of person and the environment they are in. Holistic assessment will help to identify individuality, personality and spirituality (Makienen et al, 2003). More often a nursing assessment is based on the medical side of the patient rather than the holistic approach.
Personal Perceptions Caring for terminally ill patients can be a daunting task for all involved, whether it is for the patient, friends or family, or the nurse or other clinicians. It is important to analyze one’s own feelings about disease, its progression, and death when caring for others because a patient’s quality of life and functioning can potentially be affected by the nurse’s personal attitudes. Exhibiting judgments can cause discomfort, feelings of helplessness and/or sadness, which can impede quality of life for patients. The role of the nurse is to help the patient as he or she progresses through the stages of illness, acting as a patient advocate, meanwhile providing additional support for the patient and his or her loved ones. The role of the nurse is to attempt to alleviate discomfort, restore health, and to not cause any additional undue harm.

Self awareness is a very important tool to have as it helps me to be conscious of my limits and areas for further development. It is hard to do this with a friend because we may not want the friend to know who we really are. Before we can professionally help others as counsellors, we need to be self aware. Warnings are often given to patients who do not comply with treatment or who are aggressive towards any member of staff. Although we cannot force a patient to do anything or have treatment they don’t want. We must make the patient aware that it is in their best interests that they comply with treatment. A senior doctor would have to discuss the issues regarding this with the patient in depth before a decision is made – often patients will change their mind after speaking to a doctor. Constantly reassessing the situation is vital in these types of situations as patients can often become quickly confused or distressed over the smallest of things.
If a nurse talks to a patient then conversation is confidential because the conversation is only between the nurse and the patient itself. During a one to one communication is important that the nurse speaks in simplified language so that the patient can understand and therefore there will be no need to ask again. However it can be ineffective because the message may be misunderstood or may be unclear to the person who receives the message. Furthermore communicating in a group is very effective in a hospital based setting because people are getting together in a small group who share the same problem to talk about it. In a hospital, group communications can range from AA meetings to drug abuse to various phobias which people want to overcome.
If a person becomes a drug user this becomes a physical problem that was also created due to the individual being in a bad environment, but this could also be considered a situational problem that was self induces. This type of problem can most of the time be prevented, however if not prevented it can be helped. Outpatient partial hospitalization programs (PHP) are very helpful, you are not confined to a lockdown environment as you are in an inpatient program, and your day is entirely filled with treatment which includes group therapy, one on one therapy, seeing a psychiatrist at least once during the week sometimes more, learn cognitive behavior skills that help you deal with different symptoms that may arise, occupational therapy to help deal with daily activities.
