HSC41: Use and Develop Methods and Systems to communicate, Record and Report. Communication is an integral part of care therefore its effectiveness to ensure that care is delivered as required and problems dealt with accordingly. It is about how people respond to each other in many different ways, thus it is an important requirement as a Team Leader that I understand and use good communication skills in order to develop relationship with individuals, relative, other professionals and staff. I provide active support by adhering to the policies and procedure for information sharing regarding individuals as stipulated by the organisation. For example, prior to admission, an assessment of the individual’s relevant long term social and medical history is carried out and documented, and all staff members have access to these records. A plan of care is developed based on these assessments with the involvement of the individual as his rights and preferences are what dictates what type and how care is provided. For example, Mrs G was admitted into our elderly frail unit, she is totally blind. In addition to the information we got from social services, the pre admission assessment, she was allowed to determine what time she wants to get up in the morning, what gender she wants to assist her with personal care, where she wants her bedside table to be placed, what time she wants her tea in the morning. We also get information and input from family members, friends, Social services and other key people who are involved in the provision of adequate care for the individual. Changes in care plan are documented on the automated system and passed over to staff members during the daily handover periods. Staffs are required to write in the daily diaries and report to me any conversations and observations that require adequate attention. This is done every time to indicate the
