This may be done in many different ways ie in person, email, telephone calls, text messages and written correspondence. All individuals have the right to communicate in whichever way they feel most comfortable and this should always be supported accordingly. Effective written and verbal communication and being able to use a variety of interpersonal techniques promotes strong working relationships and will promote good quality care.

Service users can be at risk if communication is not adhered to, they could deteriorate due to lack of communication or because the support worker is in a rush and is to busy to listen. Service user’s needs must always be met, communication is essential for the care to be carried out. We have senior meetings every month followed by staff meetings for all.
Nursing care is provided to every patient under the direction of a Registered Nurse which has the responsibility and accountability for the assessment, nursing interventions, delegations to PCA’s (Patient Care Assistant). For instance, nurses can delegate to PCA’s or unlicensed staff to take vital signs, to draw blood, to bathe patients, patient ambulation, etc. The manager has the responsibilities to maintain qualified staff to perform all the tasks, also is responsible for the overall budget, performance improvement, staff retention, etc. The delegation from charge nurse is going to depend on the floors where they work. Some floors allow the charge nurse to take up to two patients, and others, charge nurses only supervise and help other nurses as necessary.
I have to ensure that all important information is passed over in such a manner that I can evidence this has been done. In my role as Moving and Handling Trainer I have to communicate a large amount of information to staff whilst observing them to ensure they are following what has been taught. I liaise daily with my Home Manager either in person or via email. I also have daily contact with Head Office and have to implement all information sent to me in a timely manner. I organise all training within the home and have to ensure that staff are fully aware of courses they need to attend, when the courses are and when they have been confirmed on the course.
The support workers roles is too make sure they support the individual to there best of there ability and follow their care plan. It is the support workers role to provide the client with good quality of care. To do this you need to involve the individual in any decision making and encourage both there participation and feedback. The role of a care manager fulfils the role of a registered manager. The manager will be available to talk about any issues concerning the individuals 24 hours a day or if any problems occur.
It also ensures any health and safety issues are recognised and reported. All carers attend a hand over at the beginning of each shift and also complete communication books keeping other staff up-to-date with all current situations within the workplace. 1.2 On the job you need to communicate with people all the time. There is the service user and their family and friends, that are likely to be involved in their care. You also have to communicate with colleagues and other professionals such as doctors, nurses and pharmacists.
When communicating with service users, families and outside agency’s I use a wide range of communication methods including total communication aids to collate as much information as possible so that I can prepare the best care package for the individual and so the support team have all the information they need. This helps me keep up a good standard of communication skills and methods within my job role, this also enables me to keep up good positive rapports and relationships whilst building a trust with each individual that I work with. Communicating with other staff members ensures effective team working and continuity of care.
UNIT 4223-301 OUTCOME 1 UNDERSTAND WHAT IS REQUIRED FOR COMPETENCE IN OWN WORK ROLE. +DUTIES AND RESPONSIBILITIES. My duties are to provide clinical and non clinical support to a multi professional team which can range from the care of a new mother and baby to the day to day running of the ward. I am responsible in undertaking direct care for patients and implementing this as to prepare the patient for going home safely with the knowledge they receive. I am responsible in making sure the care of the patient has been seen through to the end of their stay and to report any observations of the patient ,to the midwife in charge of that said shift and if anything needs reporting to act accordingly,ie frequent observations.
