As if we do not clearly say how we are feeling it can lead to a misdiagnosis or not being properly treated. We also need to watch people’s body language as they can provide signals to how they are feeling if they do not wish to talk to us. Egan’s SOLER Theory can also help with communication as this can give us the basic tools to make patients feel more comfortable. When people feel comfortable they open up and can give us a better clue on the help and care we need to provide. This can also help us gain trust from the patient which can lead us to gain more information about the patients.

FACILITATE PERSON CENTRED ASSESSMENT, PLANNING, IMPLEMENTATION AND REVIEW UNIT; HSC 3020 1.1 It is important to take a holistic approach to assessment and planning of someone’s’ care and/or support. Holistic means to take into account every aspect of a person so for example not just their physical needs but how these needs affect them emotionally. This is exceptionally important as by looking at every aspect of a person’s needs I am able to provide them with the very best support and care planning that I can offer. 1.2 Whilst conducting a support plan with a tenant I support them to take the lead when planning any support needed. I do this by encouraging them to speak freely and asking open ended questions and leading questions such as “and how would you do this/that without support” and “how do you think you could manage?” I would ensure that the tenant was comfortable in their surroundings which in turn would put them at ease and enable them to talk more freely about any issues that they have enabling me to provide them with the best care and support.
Outcome 1 Understand the principles of person centred assessment and care planning 1 It is important to take a holistic approach to assessment and planning of care or support as holistic means to take into account every aspect of an individual life not just there physical of medical needs but the effects that there needs have on there emotional and social needs it is important to take this into account when planning care and support to ensure that the individual is provided with the best care to there own personal needs and preferences and also to ensure that the individual is happy with the care that the company as a whole can offer them. 2 When planning a care plan with an individual i support them to take the lead when planning any support needed by encouraging them to speak freely and ask questions about ourself as a care company ie how would this is this done? And is this possible? I always ensure that the individual is in an environment they are comfortable with and with people they want to be involved in their care planning ie family. I think this gives the individual the confidence to speak more freely and openly about their care needs and support and makes them feel more at ease to ask questions about any issues they may have with there care and support which enables me to create a care plan with the best care and support for the individual.
