1. Define person-centered values (1.1.1) Person centered values includes to treat the person as individual, recognizing their individual needs and act upon them accordingly. Support, inform and encourage them to access and exercise their rights and to make their own choices in every day life situation and in long term commitments. Making sure to give them privacy when they wish to be alone or when having private conversation. Encouraging people to be independent even if they choose riskier option. Giving them dignity and respect in all aspects of their life. When implementing all the above the carer has created productive environment and the professional relationship has changed into more equal partnership. 2. Identify possible barriers to active participation and describe ways of reducing them (4.4.2) Physical issues: Many elderly people can't manage long walks as they use to, I suggest walks in the local park or having cup of the in the local patisserie or visiting friend near by. Wheelchair users find it often difficult and uncomfortable to be wheeled on badly maintain pavements, I can suggest to use alternative route, the bus or a cab. Emotional barriers like lack of confidence can be improved gradually with constant encouragement, going out doors locally and slowly when the individual's self esteem improves expand the circle and become more adventures. Family also may be over protective. This can be overcome with good planing, telling them in advance and showing them the route. Carers may take over and for example dress the individual because is quicker but this will over time lower the client's self esteem. Over time they become more dependant and lack confidence to participate in small every day tasks even though they are capable to do so.
