Lack of effective delivery of patients and services: Due to a negative impact on the Porter services of the hospital, delivery of patients and services on time has been affected significantly. This may lead to a delay in any emergency situation and thus a bad reputation of the hospital. This bad reputation can cost a lot and can even affect their current expansion process of the hospital. Unsatisfied employees: Clearly, the staff at Greenlands General Hospital were unhappy with the current scenario. The staff included both Porter’s as well as nurses and doctors.
Some nursing positions were cut due to the demands of managed care which drastically increased the workload of individual registered nurses. Many nurses were driven away due to the growing burnout and feeling that they couldn’t meet their professional responsibilities to their patients. Other factors contributing to nursing shortage is poor work conditions, the aging RN workforce, inadequate resources for nursing research and education, the increasing complexity of health care and technology, and the rapidly aging populations as well as lack of professional autonomy. The work load is causing nurses to leave the hospital and work in other areas of nursing or other
RTT1 Task 2 Jake McKee Western Governors University RTT1 Task 2 Root Cause Analysis (RCA) that led to sentinel event Root cause in this scenario appears to be a combination of things. Most significantly, staff did not safely adjust to rapidly increasing demands of their patient acuity and census. The infrastructure did not allow simultaneous monitoring of two patients in crisis. The department is at high risk of inundation, being staffed with only one RN and one LPN, one secretary, and one emergency department physician. Secondly, balance in the monitoring of high-risk patients was inadequate.

Lisa begins her shift by following the patient’s clinical pathways or care plans to priortise her cares. Lisa is feeling concerned about Mr Bonelli and caring for him following the previous nursing staff’s comments that he was grumpy and angry. Lisa is also concerned that she has a very busy shift in her planning already and dealing with Mr Bonelli’s mood and actions could only make this worse.

Due to the high nurse- patient ratio, nurses do not have time to do charting, medicate patients on time, and provide quality care. Nurses are the professionals who empathize with the patients on physical and emotional issues. Not spending quality time with the patients may make the patients’ feel ignored and unattended. A third contributing factor is very interrelated to nursing education is that nursing education has shifted from hospital-based diploma programs to university and college programs. This shift created the need for hospitals to increase the percentage of paid nursing staff to keep up with the demand of related to the void of care provided by nursing students (Fox & Abrahamson, 2009). A fourth factor contributing to the current nursing shortage is the economic stress that nursing turnover creates in the healthcare setting. The nursing profession can be stressful mentally, physically, and emotionally creating an argument that nurses are not adequately compensated for their working environment. With other less stressful professional occupations available to a profession that is primarily female individuals are leaving the profession (Fox & Abrahamson, 2009).
Patients will not buy or take medications if they can not afford it. Many patients who have lost their jobs and health insurance are avoiding prescription drugs or office visits due to there high costs. "People who have lost jobs are putting off preventive care and canceling routine visits, (Carter, R., 2002)" which is very bad because by doing that it will create there condition into more serious illnesses. It's very important to warn patients about dosage when money is tight because some patients split pills to make them last longer. Physicians can also provide free samples, substitute generics whenever appropriate, and refer patients to pharmaceutical company or government assistance programs
During this time of bare bones staffing. Many nurses are tired. The accuity of patients does not help this situation. Many nurses bare the achiness and tiredness and push on because the patient is their top priority and deserve the best they have to give. Other nurses and staff members are not this strong, opting to go elsewhere to only do their ‘fair share’.
