Commentary. We do not know whether Maxine has a “no secrets” policy in her informed consent document. This situation would be less perplexing if Maxine has thought through the potential ramifi cations of addressing secrets and clearly stated her professional stance on the matter. Our approach is to encourage the person holding the secret to disclose it. Perhaps Maxine’s work with Tanya and her renewed commitment to couples therapy will enable Tanya to reveal this secret to Liz.
The Ethical Dilemma Regarding ‘Do Not Resuscitate’ Orders. Whilst on placement on an Adult Rehabilitation Ward, I observed an ethical dilemma. The patient concerned was issued with ‘Do Not Resuscitate’ (D.N.R.) order. This decision was reached after the consultant, named nurse, patient and the patient’s family discussed together and based their decision on the patient’s age, condition, quality of life and wishes.
It is my opinion and experience that many times when individuals are given the opportunity talk clamly to someone is actively listening, they have the ability to think rationally about their decisions, actions, etc. I would ask if she is open to marital counseling (if her husband agrees) so that she and her husband can have the opportunity to learn and utilize effective communication skills (such as actively listening to one another, giving eye contact while talking, communicate thoughts and feelings without interuption, and most importantly, learn to talk TO each other instead of AT each other) and to also address negative behaviors and/or any
Upon reading chapter twelve concerning contraception and abortion, my opinion in the matters has been altered for the more positive approach. After gaining more knowledge on ways to have safe sex as well as alternative ways of becoming sterilized, I feel that it is not a bad topic to discuss with our younger generation. When I was a teen, taking a daily pill was something I could never do. I wish I had the options that are available today. These options include and Intrauterine device (IUD) “small objects of various shapes that are inserted into the uterus” (Rathus, et al., 2011, p.367) and injectable contraceptives such as “Lunelle and Depo-Provera” (Rathus, et al., 2011, p.367).
Sadler (1991) suggests that catheter care should be carefully planned and monitored and crisis management avoided. Therefore, Individual sexuality and sexual activity should be assessed when an individual is catheterised which forms part of his or her treatment, regardless of gender or age (Atkinson 1997). However when the nurses in the author’s personal experience got to the sexuality point of the assessment, the author noticed a shortfall in the way it was conducted. The issue of sexuality was often omitted or documentation was limited to grooming and appearance. Although this could be interpretated as one end of the sexuality spectrum, the physical act of sexual intercourse could be interpretated as the other.
