Nursing incorporates holistic care while developing a care plan for patients and their support system. Holistic care looks at all of these factors and the impact the stress is having upon the patient and their support system. The nursing care plan is not only a tool for the nurse to use but also can act as a guide for the patient and family in gathering information and provide realistic expectations regarding the current diagnosis and prognosis. Nursing care plans are used whether prognosis is good, as when a patient is expecting a healthy baby without complications or when prognosis is poor, such as when a patient has terminal cancer and shortened life expectancy.
The Framework is to be used by both clients and professionals and identifies targets for both health and social care service providers to assist clients in their daily living requirements. It aims to provide more choice in care, tailored to the individual clients needs. (NSF 2004)This is particularly relevant with Spina Bifida as the condition and its symptoms vary enormously even within the same type and with a lesion in the same place. (b) Explain the importance of achieving effective communication patterns with people you work with As a carer, I must be able to communicate effectively with the people I provide care and support for to build trust and to make them satisfied with the care provided. Also communication with colleagues is important to promote good teamwork inorder to improve the health and wellbeing to the people we care for. 1.3(a) Why is it important to observe an individual’s reactions during an interaction you have with them? By observing an individual’s reaction, i can ascertain if they are happy, satisfied, comfortable, sad, worried or agitated. This tells me if everything is okay with the service I am providing to the individual in general or not.

Effective communication and interpersonal interaction between individuals is important in all situations in society, such as in health and social care settings such as hospitals, nurseries and care homes. Both theories of communication can be taken across into health and social care settings to help communication and interpersonal interaction between health and social care professionals and service users be as effective and as possible. The first theory I will be talking about is the communication cycle which i discussed in P2. The communication cycle is a type of code that requires you figuring out what the other person is trying to say and what their behaviour means. This theory would allow service users and health care professionals to communicate more easily and effectively.
It can be the key to having successful relationships both personally and professionally. As a nurse working the health care profession, having a key understanding of health care communication plays a major role in health outcomes. It is also imperative to have knowledge of therapeutic communication theories and principles as well as the relevance of effective personal and professional communication impact on the health care setting. According to The University of Boston (n.d), “Health Communication encompasses the study and use of communication strategies to inform and influence individual and community decisions that enhance health.” This plays a crucial role in the health care industry as clinical specialist, patients and families must maintain shared communication in order to sustain a positive favorable outcome of all parties. Working towards a common goal requires being able to articulate thoughts and ideas with clarity and conciseness.
Principles of health and social care practice In this essay I will be describing the principles and outlining the procedures that are needed to safeguard colleagues and service users from harm I will also state the relevance of legislation, regulations, code of practice and policies and procedures, while defining the importance of them. The theories are also relevant to health and social care; I will state what theory is and how we benefit from inter-professional working also the impact it has on individuals and services what our roles and responsibilities are in an outside work settings. The principles of care provides the foundation upon which the principles of practices are applied, values are subjective and varies from people and culture. Service users should feel valued and recognised as an individual, promoting their rights in a way that enables the service user’s choices, involving them in making decisions to meet their needs taking in perspective their rights to be treated with dignity, and have their privacy and confidentiality respected Data Protection Act (1998) states organisations are legally required to do this, information must be shared only on a need to know basis example if a service user is a danger to themselves or others even if there is a risk to the health and welfare of the care worker etc, then information must be disclosed otherwise personal records must be kept confidential, service users have the right to access information that is stored about them Access to information Act (1985) and freedom of information act (2000) suggest that. Independence must be promoted so that with support they will feel empowered to take control of their own lives, Care workers must engage constantly with diverse ethnic groups to broaden their learning to implement better skills in practice.

However there are times when the receiver may not understand the message and the cycle would start all over again. Related to health and social Argyle’s communication cycle theoryis used in health and care, as communicationskills are highly important in a health and social setting. The communication cycle is a process of sending and receiving messages between people. It is also about how the health and social worker decides how to deliver the message across. Through these skills, patients can be able to build relationships with health and social professionals and health and social care professionals with colleagues and also patients.
I believe that there is always an opportunity to help an individual and to make a positive impact on their life. I am eager to pursue a career in which I am able to help and care for people, becoming an occupational therapist would allow me to fulfill this ambition. Occupational therapy is a career for dedicated, reliable, trusting people who are able to develop relationships with patients quickly. I have these skills and I genuinely want to make a difference to people, & assist them to regain their independence. This will be achieved through a standardised approach to assessment, rehabilitation, education and health promotion.
