As a carer I demonstrate person centred values by relating to what each resident would like, what care they need, what assistance they will accept and treating each resident as a separate person but at the same time treating everyone equally. 2) A care plan is very personal to each individual as it sets out the guidelines of what care they need and receive. Carers should be able to read a care plan and understand what assistance is expected, involving a wide range of different needs. By focusing a care plan on each individual it becomes ‘person centred’, this ensures that every patient is receiving the benefits of support and assistance from carers. At St Nicholas House we review care plans at the end of each month with each resident, this ensures that any changes or issues are addressed as soon as possible.

It is important that all complaints are reported to a senior staff member or Manager and is then logged within our complaints form for investigation. All complaints received are important and must be taken seriously. The Care Quality Commission (CQC) requires my organisation to gather, evaluate and generate reports about complaints – this is to ensure the improvement of our service is ongoing and is evolving all the time. It is really important for our service users to exercise their rights as human beings.
When I’m carrying out all of these, I remember person centred values which helps me to be able to do what’s right for the client or what the client wants before carrying out any activities for the day, I go through my clients care plan but I also ask family of the clients if there has been any changes for example if my client needs to be awake and ready for a certain time, earlier or later than usual set time. I agree with them because it’s their right to change their mind. At this stage we talk together and rearrange. Personal relationships are based on emotions sharing similar attitudes, values and interests. They are also very informal, in personal relationships you share support between each other with feelings and thoughts as you can be who you are with family or friends.
Duty of care also means that all needs of the individual are being met this includes • personal hygiene • Food and water • Entertainment • Social needs • Medication. Employers also have a duty of care too, Their duty of care is to make sure that all of their employees have and safe and healthy working environment. How duty of care affects social care workers? Duty of care can affect social care workers because they have to maintain a high level of care. This is done by following policy and procedures, Being able to recognise dangers and risk withing the work setting.
It also ensures any health and safety issues are recognised and reported. All carers attend hand over at the beginning of each shift and also complete communication books after attending an individual, thereby keeping other staff informed and aware of current situations within the workplace. Service User’s communicate with carers to express their needs and preferences and to ensure they are met. As a carer I would discuss the options and choices available to the individual to allow them an informed choice regard their care. 1.2 Explain how effective communication affects all aspects of working in adult social care settings Working in health and social care is about communication and relationships.
When I am caring out all these, I do remember person centred values and this helps me to do what is right for the client or what the client wants .At the start of a shift proper handover is received, read and sign/communication book is read, clients care plan is read and follow staff meetings to get information and changes about the clients for example if I have arranged with the service user to shave during personal care ,then when the time comes he changes his mind, its his right to change his mind. Supervisions are regular, between myself and my manager. They are designed to help improve the way I work, they offer practical and guidance and support in areas that I need to improve. After supervisions are the appraisals. They evaluate what was covered in the supervisions to make sure that improvement is occurring and to set new goals. My supervisions are taken on shift, my manager will watch how I work, how I integrate with the team of staff and with the clients in the care home and put forward any ideas on what I could do differently but also explain what I am doing well.
Explain how communication affects relationships in the work setting : In care home setting communication occurs with the service users and their fammilies, the management and the directors, the working staff in the home and with outside professionals and visitors. Effective communication is vital for the support worker. The service user and the support worker need to understand each other clearly in order for the service user to receive the best possible care. Successful communication involves the support worker speaking clearly and using phrases and sentences that service users can understand. This also involves the support worker communicating clearly and openly with other members of staff, the manager and other professionals so as to make sure that the best possible care is provided and that this is done so reliably.
In a social work environment communication can be an essential tool in order to meet the needs of a vulnerable adult. It is a basic requirement in the social care role to communicate with the individuals you care for their families and also with other members of staff involved with their care on a daily basis. Communicating with the other staff members you work with shows team working and continuity of care. It also ensures health and safety issues are recognised and reported. One form of this communication is a verbal handover at the beginning and end of each shift, there is also the filling in of relevant communication books and care plans for each of the individuals you care for, this keeps others in the knowledge of the current situation in the work place with service users.
Person Centred Care Person Centred Care is a major skills acquired by a healthcare providers.Which main target is individual traits of character in doing health care provision. Treat every person as a unique human being disregard his/her age, culture, sex and race.Acknowledge, respect, and take into considerations the choice that every service is entitled to. Set some standards for practice but not so precise to deny the specific application demands of each individual uniqueness. Guidelines might be essential for the care providers to include complex concerns that help the nurses but sometimes leads to apart a patients' rights. According to Professor Draper ,getting to know the person behind the illness is the key principle of person centred nursing care.We as the care providers needs to listen diligently to our patients conditions, treat as a unique human being.
