Samuel Gallagher is a 16 month old male child from New Prague, Minnesota. He is in overall good physical health and is of average stature. Sam lives at home with his nuclear family consisting of his father and mother. Sam’s father works full time and his mother works part time, therefore he does spend approximately 10 hours per week with an alternate care giver. This interview was conducted with Sam’s mother present, in various areas of Sam’s home as to accommodate testing and observations. This interview took approximately 45 minutes to complete. List of Questions 1. Physical assessment of subject. 2. How would you describe Sam’s temperament? 3. Can a general timeline of Sam’s nutrition be established? 4. What is Sam’s preferred method of ambulation? 5. What method of play does Sam engage in? 6. What is Sam’s reaction to strangers? 7. Has Sam developed object permanence? 8. What methods does Sam use to communicate? 9. Does Sam display self recognition? 10. Is Sam able to feed himself using a spoon/fork and drink from a cup? 11. Does Sam enjoy crayons/markers and can he scribble? If he can scribble, does he use a full arm movement when coloring? 12. How does Sam handle multiple objects? 13. Is Sam exposed to reading and/or books? 14. In what manner is affection shown to Sam and how is it reciprocated? 15. What is Sam’s reaction if he becomes unsure of a situation? 1. Physical assessment of subject. Sam was born September 15, 2010 via c-section due to breech presentation. As of his last doctor appointment, Sam’s height is 31 inches, weight is 24 pounds, and has a head circumference of 19.5 inches.
