Date of Birth: 2/25/69 Age: 44 Ethnicity: Caucasian Education: Doctorate Gender: Male Occupation: Manager at Holland Interior Marital Status: Separated Dates of Evaluation: 3/29/13 Date of Report: April 13, 2015 Name of Evaluator: Shaurice Allen REASON FOR REFERRAL: T.M. was arrested in Cook County for Aggravated Criminal Sexual Assault on June 17, 2012. He then was admitted to Elgin Mental Health Center (EMHC) for the Forensic Treatment Program on January 10, 2013. Per the request of his public defender, T.M. was referred by the Honorable Judge Joe Smith of the Cook County 16th Circuit Court for a Competency to Stand Trial (CST) evaluation. The evaluation was conducted to aid in determining TM’s fitness to stand trial. TESTS ADMINISTERED: 1. Clinical Interview 2. Minnesota Multiphasic Personality Inventory – 2 (MMPI-2) 3. Million Clinical Multiaxial Inventory-III (MCMI-III) 4. Structured Interview of Reported Symptoms, 2nd Edition (SIRS-2) BACKGROUND INFORMATION: (The following information was obtained through a clinical interview with T.M. and through a Psychological Services Unit File Review. There were several inconsistencies between T.M.’s self-report and his records so information should be evaluated with caution. T.M. was informed about the purposes of the evaluation, limits of confidentiality, and that the report would be submitted to the court.
