Engage in personal development in health and social care My role as a community support worker entails primarily two general areas. Firstly, to provide care for the service users and secondly to provide support services in relation to the individuals that I work with. By providing care, there is a need to provide it with warmth and professionalism by meeting the various support needs of service users whilst promoting their independence as much as possible. The care and support may include but not limited to; personal care, medication administration, physical and mental health activities, community participation as well as continuous assessments such as risk assessments. Additionally, maintaining accurate records of all care and support given is a requirement of my job role.
Person-centred care involves: • Compassion, dignity and respect – these are the essential foundation for the greater involvement of people in their own care. • Shared decision making – this requires the involvement of patients as equal partners in their healthcare. Tools to support shared decision making can include self-management support, access to personal health records, personal health budgets, care planning and shared treatment decisions. • Collective patient
It represent a list of statement about proffesional standards and practice that is required in daily work in the health care setting. Also Codes of Practice ensure social care workers to know the standards of conduct about their collegues, other employers, carers, service users and mainly what public expect of them. They have to support the standards of the code that is thet and to make sure that they meet their code. Following the requirement of the code a carer worker must protect their rights and to promote the interest of the person he takes care of. They have to treat every person as a an individual personality to respect and support his wishes.
The care plan will make sure all needs applying to person centred values are understood, it acts as a guide for carers, so it is clear what sorts of activities are expected of them, this should be updated with the SU's changing needs. There should be regular reviews, It is a process of continually listening and focusing on what is important to the SU now and in the future, the individual and you should be involved in discussion about how it is working and whether any parts need changing and if the SU's needs are being met. It is vital that we make sure that the SU is central throughout the process, we must gather all relevant information about their life and needs. Very often you will only be caring for and supporting people when they are in a vulnerable position. The quality of care that you can provide will be improved if you have knowledge of the whole person, not just the current circumstances, this knowledge can help us better understand why people may behave in a certain way.
Unit 1: Promote communication in Health, Social Care or Children’s and Young Peoples Settings. Identify the different reasons people communicate. . Communication is a basic human need. People communicate for a variety of reasons.
Unit 7: Implement Personal-Centred Approaches in Health and Social Care 1.1 Person-centred values means the individuals whom we support are involved and included in every aspect of their care and support. For example rights, individuality, choice, privacy, independence, dignity, respect, partnership and autocracy. 1.2 Why it is important to work in a way that incorporates a persons centred values? Person centred methods, plans and processes, as well as care carried out puts the person being supported at the center of the day-to-day activities. Family members are also included in planning and maintaining, which means its personal and private to the people whom really know the person.
1.1 Personal centred values must influence all aspects of health and social care work. This is because it involves the individual that is centre to the care by considering all aspects of their life, including their rights and personal preferences (including religious beliefs) as well as maintaining privacy, dignity, Independence and respect. By doing this it will conclude to the best possible care for the individual who receives it and it will empower the individual as well as include them too. 1.2 personal centred care plans are much more detailed than basic care plans. As well as the necessary assessment of needs and circumstances, required outcome and timescales for carrying it out and reviewing; personal centred care plans will include much more information about the individuals desires relating to their care.
All of these work towards the same basic values. Which are: • Treating people as individuals • Supporting rights • Privacy • Independence • Treating people with respect and dignity • Working together as a partnership. All of the above are the basic foundations to empower individuals. 1.2 Explain why it is important to work in a way that embeds person centred values. It is important to work in a way that embeds person centred values as this will establish that the correct criteria is in place to support the individual and to meet their needs and goals, whilst still empowering the individual to be in control of what type of support that is being delivered.
Providing support for vulnerable individuals should always be aimed to respect privacy, rights and choices and recognise the importance of individual values and diversity. This also highlights the importance of communication in awareness of the individual’s needs, their preferences and choices. Empowering individuals to make choices and decisions about their own life and to set and achieve goals means an independent life can be supported. 2.1 Communication is essential in person-centred approaches as it is a vital tool is assessing an individual’s preferences and choices as well as the needs they express. This may include sharing decision making with the individual being supported to ensure their choices are incorporated into the care and support they receive.
Unit 1 – communication and supportive skills Communication is important in almost all fields. Communication helps you to understand what the other person is saying. It is also an important strategy in order to deliver what you or the other person is trying to say. The role of communication is to develop new skills and clearly understand the service user in order to give the best possible health care. As a health professional if you want to deliver good quality care you need to communicate effectively and it will also help you to learn the needs of your client this is important because so that the service user knows they are in good hands and it also gives them an idea on what is going to happen.
