Safety, Communication and Placement of the Older Adult Kellie Passaretti Western Governors University Safety, Communication and Placement of the Older Adult Healthcare Issues and Their Importance Mr.Trosack has many issues that will need to be addressed for a successful discharge plan to be put in place. These issues include: 1. Decreased mobility related to his total hip replacement (THR). Decreased mobility often results in decreased ability for self-care, decreased ability to maintain a social network and a decreased will to improve. “Decreased activity can increase joint stiffness and weakness, further compromising mobility” (Mauk, 2010, p. 463). 2. The need for weight loss and diabetes control. It is important to establish long-term management of diabetes to prevent complications. These complications include heart disease, stroke, circulatory problems and visual problems. The best way to approach this management is through appropriate medications, diet and exercise (Mauk, 2010). 3. New onset hypertension. The greatest risk factor for stroke is hypertension (Mauk, 2010, p. 393). It is therefore imperative to maintain appropriate blood pressure control. 4. Possible depression due to loss of independence, denial of current health issues and lack of social support. “Elderly men have the highest rates of suicide in the nation” (Mauk, 2010, p. 363). The loss of his fiercely held independence, as well as being a widower, makes Mr. Trosack very vulnerable to depression (Mauk, 2010, p. 341). 5. Lack of family support due to denial of his current health issues and their busy lives. This lack of support could result in his non-compliance to the appropriate plan of care resulting in delayed or deferred health improvement.
