T.M. was informed about the purposes of the evaluation, limits of confidentiality, and that the report would be submitted to the court. He acknowledged his understanding and agreed participate. ) Family History T.M. was born and raised in Missouri until the age of 12, where he lived with his mother and stepfather and siblings. T.M. reported that his biological father was “schizophrenic and abusive”, and that he sexual and physically abused T.M. during his childhood. He was married in 1995, at age 26, to a woman who was 22 at the time. They were separated in 2004, when T.M. was 35, in which he moved in with his mother due to the separation. T.M. and his wife have been legally separated for the past 9 years and he stated it’s because, “We’re Catholic. No divorce.” They had 6 daughters and 1 son who all, but the second oldest daughter, currently reside with the biological mother. should be referred to a psychiatrist for an evaluation to determine his need for medication. Individuals who display affective and psychotic symptoms usually benefit from a combination of psychotherapy and medication management. T.M. appears to be undergoing episodes of depression, and hypomanic or manic episodes. He also has displays symptoms of psychosis manifested in his delusional and grandiose behavior as well as his self- reports of hallucinations. Thirdly, T.M. should be admitted back into a substance rehabilitation program. He has undergone rehabilitation programs before, but it seems as if he was unsuccessful as seen by his continued usage of alcohol, cocaine, and marijuana. These unsuccessful attempts may be attributed to his psychological issues in which he was using the substances to cope with. Once his mood is stabilized and he learns better coping and social skills, it is likely that T.M. will be more successful in attempts to refrain from substance
