EVIDENCE BASED RESEARCH INTRODUCTION Evidence based clinical decision making is effective and crucial in ensuring positive outcomes in diverse clinical settings. According to Burns and Grove (2010) evidence based practice is paramount in the delivery of high quality, cost-effective care and patient values. In addition, nurses are obligated by the Nursing and Midwifery Council (2008) to adhere to best available evidence as it maximizes effects of clinical judgement. It is essential for nurses to have the knowledge of critiquing research to deliver enhanced care on patients (Royal College of Nursing 2012). This assignment, written as a report, will focus on obesity, paying particular attention on moving and handling.
This paper will critically review how Sister Jones could improve the privacy and dignity practice on Vauxhall ward using effective change management and change strategies theories with focus on factor 3 of the Essence of Care Privacy and Dignity Benchmarks (Department of Health (DOH), 2001a) (Appendix 1). By using Lewin’s Theory of Change Model (Cited in Baulcomb, 2003), Action Research (A.R. ), a process of organization development, will be explored to assist in the required change. (McNiff and Whitehead, 2006) describes A.R. as an enquiry that allows practitioners to constantly refine practice and continue their professional development using the evidence based outcomes.
In this part of this assignment I am going to critically analyse an article by evaluating the relevance of the findings from the research. The article is “A Qualitative study investigating patients’ beliefs about cardiac rehabilitation.” I am going to use the Critical Appraisal Skills Programme (CASP) to aid my critique. The CASP tool systematically explores the research to judge its value, relevance and trustworthiness. My evaluation on this evidence is that it has many good points and many bad points. I am going to focus on some of the good and bad points by critically appraising this article.
Ethical Issues in Nursing This book examines major ethical issues in nursing practice. It eschews the abstract approaches of bioethics and medical ethics, and takes as its point of departure the difficulties nurses experience practising within the confines of a biomedical model and a hierarchical health care system. It breaks out of the rigid categories of mainstream health care ethics (autonomy, beneficence, quality of life, utilitarianism…) and provides case studies, experiences and challenging lines of thought for the new professional nurse. The contributors examine the role of the nurse in relation to themes such as informed consent, privacy and dignity, and confidentiality. Nursing accountability is also considered in relation to the contemporary Western health care system as a whole.
The practice of evidence-based medicine means integrating individual clinical expertise with the best available external clinical evidence from systematic research" The whole purpose of EBP is to use available research to inform practice and as a result of good judgement by health care professionals to ensure we do what is best for our patients. The Nursing and Midwifery Council (NMC, 2008) state that all advice given to the patients is based upon the best available evidence based practiced. Research Process: Components and Validity According to Burns & Grove, Nursing research is a scientific way of validating, and improving existing knowledge and creating new knowledge which can affect nursing practice (McEwen &Wills, 2011).The researcher will use various methods and options to explore the answers for Research question and this process is flexible and circular (Fain, 2013).Because, the researcher may have to continuously review the literature and analyze the concepts. Results of a research study can be used for the initiation of another research (Fain, 2013, p.19).General steps or components of research process are 1.Selecting and defining the problem, 2.Selecting a research design, 3.Collecting data, 4.Analyzing data, 5.Using theresearch findings. (Fain,2013,p.20) Selecting and defining the problem: In this step, the researcher will identify a problem which will help them to improve the existing knowledge. Review of Literature is an essential part of this step .The researcher will identify pertinent variables, define theoretical frame work, formulate research questions and hypothesis (Fain, 2013). Selecting a Research Design: In this step researcher will select a design for the study in which sample selection, setting and method of measurements or tools are included.
Dorothea Orem-Self Care Theory Dorothea Orem- Self Care Theory Dorothea Orem’s theory of self-care is based on three related theories: the theory of self- care, the theory of self-care deficit and the theory of nursing systems (Roussel, 2013). Dorothea Orem’s theories have become a central point in the progress of curriculum for numerous schools of nursing (Flynn & Heffron, 1988). According to Orem, it is the special focus on human beings that distinguishes or differentiates nursing from other human services (Nursing Theory, 2013). Orem’s viewpoint of nursing is centered on each person’s self-care agency. She defined self-care as, “the practice of activities that individuals personally initiate and perform on their own behalf in maintaining life, health, and well-being” (Nursing Theory, 2013).
The following essay aims to identify how reflection in action and on action may impact on the provision of services by District Nurses. The author felt it important to define the two concepts, in order to understand their relevance to practice. It was found by Schon (1983) that reflection- in-action, was a nurses’ ability to understand how an individual was responding to a nursing intervention at the present time; in contrast to reflection-on-action which was thinking through a situation after it had occurred. From this understanding the author established through in depth reading that reflection has been a topic of discussion within nursing, health and social care professions for years (Kinsella, 2009). The theory of reflection has made the author identify the epistemology of reflection in and on action and the effect this can have on her professional growth.
