1.Assessment: Patient is assessed upon entering the ED by the admission team for signs and symptoms of MI, CVA or other life threatening conditions. Patient is moved to triage where they are assessed more thoroughly, and once they are in a bed in the ED they are assessed continuously.
Diagnosis: Patient is diagnosed by nursing staff upon assessment in admission, and triage, where nursing diagnoses are given such as acute pain, anxiety, or impaired tissue integrity. Once the physician assesses the patient an actual medical diagnosis is given, or labs/ imaging are ordered to aid in the diagnosis.
Planning: Planning is done by nursing staff and physician before a diagnosis is made to control or ease presenting signs and symptoms. Further planning is done once accurate diagnoses are made.
Implementation: Implementation of these plans is carried out as soon as the plans for care are made. If the plan is to admit the patient they are moved to a bed on the floor as soon as possible, so further care can be given. Physician implements his plan by ordering meds and other diagnostic tests, the RN assists the physician in filling these orders for treatment while also implementing the plans from a nursing care point of view, and the techs on the floor help the RN in any way possible.
Evaluation: Evaluation is done continuously as each part of the plan is implemented. Evaluation of lab values and diagnostic imaging is done as soon as they are available; evaluation of meds given is done once meds have reached their point of onset, and so forth.
2. Many different roles are played by the RN in the ED. They are assigned to areas such as triage nurse, staff nurse, nurse for a particular section, or tech nurse each day and specialized knowledge is required for each position.
