Whenever a patient is admitted to hospital through Accident and Emergency it is essential that a number of baseline observations are taken in order to determine a patients progress whilst staying in hospital. The physiological measurements that should be recorded are respiratory rate, oxygen saturation, temperature, blood pressure, heart rate, and level of consciousness. Once a patient has been admitted to a particular ward, it is the responsibility of the Medical and Senior Nursing team to develop a monitoring system tailored to each individual patient in relation to their specific medical needs.

On admission to hospital the patient is fitted with an identification tag which is usually placed on the patient’s wrist this should include their name, date of birth and patients hospital number. Before the patient is called to the anaesthetic room all anaesthetic equipment and machines must be checked and a record of the name of the person that checked it must be recorded along with the date. All other machines such as diathermy and suction machines must also be checked as well as other electrical equipment and theatre room lights. All drugs used for the planned case will also then be checked and prepared ready for the anaesthetist (Aitkenhead 2007). Upon entering the anaesthetic room the patient may be feeling a little venerable and unsure of what is happening therefore reassuring the patient and having good communication skills is important we should introduce our self and be able to explain what is happening and why, in a way that the patient understands (Oakley & Limborgh 2005).

An LPN will work under the supervision of doctors and RNs to assist patients in the recovery process. They take patients vital signs, administer shots, help doctors and RNs with some procedures, as well as assist in bathing and feeding patients in a hospital setting. They are also allowed to take samples for laboratory test. In an office setting, an LPN will also make appointments for the doctor and perform some clerical duties as well. This time frame is called the orientation period.
