Managing complex medical issues for patients with Parkinson’s disease involves applying previous knowledge or wisdom in treatment (McGonigle & Mastrian, 2012). Continuum of care approach uses previous wisdom gained in outcomes to deliver patient care through a defined period of time. Better coordination of patient care helps to produce best possible outcomes. According to Laureate Education (2010) nursing informatics is a major player in continuum of care and it consists of data, information, knowledge, and wisdom.
