Decision-Making Case Study Kristie Marion HCS/514 May-Fourteenth, 2012 Michael Curtis Decision-Making Case Study This paper will outline a case study of a county clinic serving a predominantly Medicaid population. The clinic is experiencing a 15% decrease in budget. The constraints of the budget force the manager to decide what clinical services to eliminate or introduce optimally to serve the needs of the customers in this community. It will cover that of the six tools in the Informed Decisions Toolbox (IDT) used to make the decision regarding what services are maintained. It will describe the rationale for the use of this tool and analyze the tools effectiveness. Finally, it will describe the effect of using the toolbox on accountability, knowledge transfer, and a questioning organization. Demographics The location of the county clinic is in Los Banos, California, a city in Merced County with a population of 35,972. The racial makeup of Los Banos is 64.9% Hispanic or Latino, 26.5% White non-Hispanic, 3.8% Black, 3.2% Asian, 1.4% American Indian, and .4% Pacific Islander. 55.7% of the populations speak a language other than English at home and 18.7% of the population in this community is below poverty level (U.s Census Bureau, 2010) Medicaid is the predominant health insurance of the patients at the clinic. Background Currently the clinic provides services 12 hours a day six days a week by appointment or walk-in. “It offers urology, surgical, orthopedic, ENT, gynecology and child development services to patients who qualify” (Memorial Hospital Los Banos, n.d.). It also offers education as part of its preventative measures and promotion of health. Staffed in the clinic are three nurse practitioners, two Registered Nurses, two Licensed Vocational Nurses, and five clerical staff. On average the clinic is seeing two patients an hour.
