Controlled Substance Record Keeping The regulations for recordkeeping of controlled substances are the responsibility of the registered practitioner. The registered practitioner is not required to keep records of controlled substances that have been prescribed in the lawful course of their professional practice unless said controlled substances are prescribed for detoxification program or a maintenance program. The registered practitioner is required to keep records of any controlled substances that were administered in the course of detoxification treatment or maintenance program of an individual (Section VI – Record Keeping, n.d.). On a current basis, and for each controlled substance purchased, dispensed, distributed, stored, received and otherwise disposed of, every pharmacy must maintain accurate and complete records (Section VI – Record Keeping, n.d.). The records that are required to be maintained by a pharmacy include the following: 1. Records of controlled substances that have been distributed through returns to vendors, sales to other registrants, and distributions to reverse distributors. 2. Any and all receipts and/or invoices for any schedule III, IV, and V controlled substances. 3. Records of all controlled substances dispensed such as prescriptions and schedule V logbook. 4. Any records of transfers of controlled substances between pharmacies. 5. All inventory records of controlled substances dated as of beginning or close of business, including the initial and biennial inventory records. 6.
