This piece of written work aims to discuss the principles of end of life care in a care home environment along with palliative care. This will incorporate the Gold Standards Framework. The author of this piece of written work is a registered learning disability nurse working in a nursing home for residents with learning disabilities. End of life care is provided within this home for nursing and residential residents. End of life care is provided within the home when all necessary measures are implemented. End of life care “helps all those with advanced, progressive, incurable illness to live as well as possible until they die. It enables the supportive and palliative care needs of both patient and family to be identified and met throughout the last phase of life and into bereavement. It includes management of pain and other symptoms and provision of psychological, social, spiritual and practical support” (National council for Palliative care (2006), cited in: DH, 2008, p47). Clients that the author has cared for in a generalist manner have had progressive illnesses, such as dementia or cancer for example. Treatment is incorporated into client’s care plans to allow all staff to follow the same treatment in the same manner to maintain continuity of care. Treatment is reviewed by evaluating care plans and discussion with other members of the multidisciplinary team and medication is increased or changed with consultations with G.P.s. Palliative care is defined as “holistic care that is not preventative or curative but addresses symptoms associated with the condition” (Mencap, 2008, p.11). Within the nursing home that the author works in, palliative care is carried out on a 24hour basis. For clients with dementia for example, staff carry out care interventions round the clock as many of the clients wander around the home during the day and night and
