To be able to do this there needs to be clear objectives set which must be worked towards in order to reach the desired outcome. To be able to work with outcome based practice the desired outcome must be established first, this helps the team, service users and others know what the end product should be. Throughout this process there should be regular reviews to find out how outcomes are being met and find out the impact or changes that it had had on the service user. Where positive changes have been identified the programme should continue towards the goal, if however there appears to have had a negative impact on the service users then new strategies should be discussed. It follows a person centred approach to support, always keeping the needs and wishes of the service user in mind and looking to achieve the best possible outcome.

Process of goal settings Goal setting is a powerful process for thinking about ideal future and motivation to make this future vision into reality. This process helps you make a plan about what you want to do in the future, that helps you organize your time and resources so that you can exploit the opportunities of life. Goal setting is an important method of: -Deciding what you want to achieve in your life. -Separating what's important from what's irrelevant, or a distraction. -Motivating yourself.
When solving the crisis of time mismanagement several questions necessitate being asked, then investigated: What strategies are used for organizating time successfully? What are the rewards of managing time sensibly? How can anxiety be reduced? Some necessary strategies for managing time sensibly are having self-knowledge and goals. Possessing an alertness of my goals will help in prioritizing tasks.
A goal is what a person is trying to accomplish (DuBrin, 2004). In order to direct ourselves we set goals that are clear and understandable, challenging, and achievable. Feedback is an important element in goal setting. We need feedback so we can determine whether we are succeeding or whether we need to change our goals and/or direction. We find feedback very encouraging and motivating.

There are also many different theories that revolve around how and why a person is motivated to behave in such ways. Motivation “is to be moved into action” and these motives are influenced by “environmental incentives” or an inner drive (Deckers, 2005). If one does not have motivation, one will give up at the very first sign of difficulties or adversities. Motivation also means to be inspired and to be able encourage others to do their best at what ever task they try to accomplish. Motivation will help induce an individual to think "If I am unable to do something or if I cannot, then I must put forth an effort and try."
So at the first glance, it may seem that they are two different words with same meaning. Let's take a closer look. Goals are milestones, significant events towards the Vision while Objective is a specific point in a plan or a waypoint on our journey. Objectives are a bit more specific than Goals; they are more near-term, and perhaps tactical. Objectives are specific, require skill, and are characterized in the actual tasks we carry out in pursuit of our goals.
The first one is that individuality means more than claiming independence, it means achieving it by acknowledging the influences that have shaped my thinking, by sorting and evaluating my ideas and attitudes, and finally by choosing the best ideas by resisting the pressure of habit and by changing the ways that I think because the evidence tells me to do so. The second subject that was significant to me was how to distinguish a problem from an issue and how to solve them. Solving a problem means deciding what action will change the situation and make it better, whereas solving an issue means deciding what belief or viewpoint is the most reasonable. The third subject was discussed in chapter 8 and taught us how to investigate a problem or issue. According to Ruggiero (2012), “It means getting information others overlook by searching in ways and places that never occur to the uncreative.” (p.138).
