Enhancing Patient Safety by reducing Medical Errors I chose the topic of “Enhancing patient safety by reducing medical errors” because it is a topic that I am very interested in. No matter what area of nursing I choose to go in, patient safety is always going to be a crucial part of the healthcare system. I know that when I work with the different patients, I am going have to provide a safe quality of care, be aware of the different risk factors that can affect a patient’s safety. I understand it is going to be a challenge to keep patients safe because errors and mistakes happen every day, and they can happen at any time. I feel like my job as a nurse would be to make sure that no errors occur, and that the patient is safe at all times.
A professional nurse is one who puts the needs and importance of patient care above all others. The professional should exert a positive public image considering the public is where services are directed to. A professional appearance is important for making first impressions on patients and other coworkers. The first impression is always of utmost importance and plays a major role in gaining a patient’s respect and trust. Once the nurse has began establishing a relationship with their patient, they must be careful to not pass or show any judgment on a patient’s decisions, beliefs, values, or culture.
Effective communication is important to get information from patient or staffs. Actually experience, skill, condition of the patient, health care environment, and the system of communication plays an important role for the treatment of the patient. Communication skills are crucial in the health care setting. Usually right after we visit the patient for the first time, we can figure out his or her mood and general health condition. According to that we can investigate the problem and take action to resolve it.
Why is reflective practice important? Reflective practice is incredibly important in a health and social care career, mainly due to having to care for anyone and everyone who needs it. You may not see eye to eye with some colleagues/clients you care for but committing to reflective practice will help you realize that opinions and ideology needs to be put aside to get to professional level of care. How reflective practice contributes to improving the quality of service provision Reflective practice contributes to improving the quality of service provision as the care sector are always assessing themselves by reflecting and evaluating on their previous work to help identify each care workers strengths and weaknesses. Reflection and evaluation are a necessity mainly to determine which areas you are excelling in and which areas you may require more improvement and more of your time.
Leadership and management in nursing are very important, which indirectly affect the quality of health care provided to patients. Donner and Wheeler (2004) stated that, leadership and management are believed to be required at all levels of the organization to provide staff alignment, learning and professional development, which can help in optimizing the patient's positive. In other words without both parties everything can be failure. Prosperity in health care can lead to optimal patient outcomes through the provision of employee organizations, which in turn achieve the vision and mission set by the organization. To describe what are management and leadership, Kolter (1990), describes that management is to prevent chaos through structured policies and monitoring, beside that it is to provide clear goals and direction to allow others to achieve their policies, objectives, vision and mission.
In order for the health care manager to assure that operations run smoothly, I feel that planning is the most important role of these four roles. Within the role of planning a health care manager can make essential decisions concerning their facility. This allows them to identify preferred work results that can be achieved by staff with positive outcomes. Suitable decisions demand perspective-a clear, comprehensive view of actual performance. Such perspective demands are necessary for the planning of continuous performance improvement.
The second part or the domain of the accountability refers to what activity must be held accountable to the party. For example, competence, misconduct, legal, and ethical issues are domains to be held accountable for. The third part of accountability which is the procedures of accountability refers to the procedures both formal and informal. Accountability is important in every industry, however accountability is necessary in the health care industry to ensure the highest quality of care is given to patients. This paper will address why accountability is so important in the health care industry.

Bulky or ambiguous guidelines are poor clinical risk management tools (Wilson & Symon 2002). ‘Clinical governance is the system through which NHS organizations are accountable for continuously improving the quality of their services and safeguarding high standards of care, by creating an environment in which clinical excellence will flourish’ (DoH 2008). Clinical governance aims to improve quality of care by getting things right first time and every time. One of the key elements of clinical governance is Audit.
