Quality Improvement Plan: Quality Data Collection
Quality improvement is not a choice but a necessity for health care organizations.   In order for these quality improvements be successful, quality data must be collected and analyzed.   In part two of the quality improvement plan, the focus will on the tools available to health care organizations to maintain quality data collection for the meeting of the Centers for Medicare and Medicaid (CMS) perfect care bundle core measures and reducing the rates of readmissions at the health care facility.   The focus will include three data collection tools that can be used to collect performance information, the types of information each tool collect, the strengths and weaknesses of each tool and, how the data collection tools are similar or different.   The second section of the paper will include two tools that measure and display the quality improvement data gathered from the data collection tools, types of information that each tool measure and display, each tools strengths and weaknesses, how the tools are similar or different and, how these tools are helpful for health care organizations.
Data Collection Tools
While there are several data collections tools available, this section will focus on; PDCA, billing and coding and, process mapping.   “Quality measures, which are required by regulatory agencies, can offer health care leaders information to access and improve patient care and to ensure that they have timely, efficient and effective care, with expected outcomes” (Dlugacz, 2006, p. 65).   The Plan, Do, Check and, Act (PDCA) is a data collection tool that the health care organization can use to monitor quality improvement, standardize processes which reduces variations which eliminates errors and rework of the data collected.   The measurements obtained are...
