Several of those issues began in the 1990s when changes in the health care system were implemented. Due to conflicts in funding, hospital staffs were greatly reduced. When facilities required more nurses such as times when there was an unexpected increase in patient admission, instead of hiring additional staff, mandatory overtime was used (Keenan, 2003). At the height of their need, the amount of practicing nurses has declined. With baby boomers reaching retirement age they will need health care services which only increases the demand for nurses.
According to the U.S. House of Representatives, majority of nursing homes do not have enough staff to meet the levels recommended by federal officials; the levels recommended are 3.45 nursing hours per patient daily. Senior’s usually lose their primary care physician, who is most familiar with the patient’s medical history and conditions, when they enter a nursing home. As a result of entering a new living environment, elders are given a doctor who has minimal knowledge when it comes to their medical history, and unfortunately physicians in nursing homes have an overload of patients and are constantly in a rush. Under federal law, physicians in nursing facilities are only required to see the resident once every thirty days for the first ninety days after a new patient is admitted to the nursing home. After the
The obstacles to effective delegation can be summarized as belonging to one of three general groups: communication, interpersonal relationships, and knowledge. Ineffective communication impedes effective delegation since delegation relies on sharing findings and knowledge (Finkelman, 2012, p. 412). Weak interpersonal relationships foster a lack of trust and confidence, fear of disapproval or of losing control, or feeling that that no one can perform a task as well as you (Finkelman, 2012, p. 412-413). I witnessed a nursing tech who has been employed by my hospital for 30 years, telling a young new nurse “the
For the past six years the media have been reporting a shortage of registered nurses (RNs) in the United States This shortage is most severe for hospitals, in western and southwestern states, in New England, and for specialized advanced practice nurses. Estimates of average nurse vacancy rates at hospitals range from 10.2 percent to 13 percent, with one in seven hospitals reporting more than 20 percent. (Projected Supply, Demand, and Shortages of Registered Nurses, 2000-2020) At PFCH the Chief Nursing officer has approached the President requesting that some consideration be taken into hiring more nurses. It has been mentioned that the nurses are over worked and it has begun to take a negative effect on patient care and frustration among the
Insomnia is the disruption in the amount and quality of sleep that impairs functioning (Ackley, 2014) linked stress and depression. P: represents the plan. Based on the assessment and diagnosis, the nurse sets measurable and achievable short- and long-range goals (American Nurses Association, 2014) For example, A short term goal for a patient with insomnia would be to achieve X amount of hours of sleep per night over the course of a week. Next is I: which stands for Implementation. After the nurse has assessed, diagnosed, and made a plan for the patient it must be implemented.
Staffing issues Sandra Terry Drexel University Critical issues facing most hospitals and long term facilities today is staffing correctly on nursing units for the number of patients and acuity of patients. What happens when the nursing units are not staffed adequately from budget cuts, illness or nursing shortage, and what can be done? When units are not properly staffed, there is an increase in mortality nurses run the risk of medication errors, missed charting, decreased patient satisfaction, and the possibility of having a hospital stay longer than necessary. These issues are usually the result of nurses unable to take the time needed to assess their patients. When there are more patients to handle than time allows nurses might take short cuts in their nursing care.
Scarce Resource Article Hazel Hofer NUR 531 September 16, 2013 Tracey Lane Scarce Resource Article Registered nurses (RNs) are an important part of the health care environment, without them delivering quality healthcare would be very difficult if not impossible. The nursing shortage was originally projected that by 2020 there will be approximately a 36% nursing vacancy rate in the United States (National Center for Health Workforce Analysis, 2013). A more recent article estimates a nursing shortage of approximately 300,000 to 1,000,000 nurses by 2020 with the largest deficits in California, Florida, and Texas (Juraschek, Zhang, Ranganathan, & Lin, 2012). In the 2008 book written by Buerhaus, Staiger, and Auerbach it is estimated
